About The Position

The Director of Care Management oversees all aspects of program development, implementation, and monitoring ensuring compliance with business, state, and federal guidelines. This role involves recommending, developing, and executing initiatives while evaluating outcomes to enhance care management performance. The Program Director collaborates closely with provider partners and stakeholders to drive continuous quality improvement.

Requirements

  • Master’s Degree in a related field (such as Social Work, Counseling, Clinical Psychology)
  • Minimum of three years post-master’s related supervisory experience supporting children with mental health needs
  • Clinically and culturally competent/responsive, with the training and experience necessary to manage staff and complex family needs across child-serving systems
  • Valid and Current Driver’s License
  • Maintenance of license in appropriate discipline (LAC/LPC or LSW/LCSW)
  • Leadership and Management: Ability to motivate and guide teams towards achieving program goals.
  • Project Management: Strong organizational skills to manage multiple tasks and meet deadlines.
  • Analytical Skills: Capacity to interpret data and use insights to inform program improvements.
  • Communication Skills: Effective verbal and written communication to engage with diverse audiences.
  • Budgeting and Financial Management: Understanding of financial principles and ability to manage program budgets.

Responsibilities

  • Developing program goals, objectives, and strategies aligned with organizational priorities.
  • Creating program curriculum, structure, and delivery methods.
  • Hiring, training, and supervising program staff.
  • Managing program finances, including allocating resources and monitoring expenditures.
  • Overseeing the delivery of program activities and ensuring adherence to timelines.
  • Monitoring program effectiveness through data collection and analysis, making necessary adjustments.
  • Building relationships with key stakeholders, including funders, community partners, and program participants.
  • Providing regular updates on program progress to leadership and stakeholders.
  • Assisting COO in supervising business functions as it relates to service delivery.
  • Creating relationship-driven people initiatives.
  • Building trust and partnerships with interdepartmental subject matter experts.
  • Leading and managing the quality of in-person care management.
  • Working as a liaison with external organizations to ensure lines of communication are in place for member engagement.
  • Monitoring all work product to ensure it is in compliance with all State and/or Federal mandates.
  • Evaluating departmental operations with a focus on process improvement.
  • Continuously improving effectiveness and efficiency of processes and program.
  • Aligning services and care delivery to the strategic plan.
  • Additional duties as assigned.
