The Director of Care Coordination will plan, implement, and administer comprehensive service delivery in case management, progression of care, social work services, discharge planning, and transitions in care. This role involves promoting and establishing effective communication between Care Coordination and other Hospital departments and members of the medical staff. The Director will stay informed about current regulatory and professional trends, changes, and standards applicable to the scope of Care Coordination, making appropriate recommendations or changes in policies, procedures, and/or programs to conform to regulation changes and standards. They will also keep staff informed of Hospital Organizational goals, strategic plans and initiatives, finances, and Senior Management communication to ensure identification with the overall Hospital mission and goals. With Managers, the Director will establish effective lines of communication for the completion of objectives, oversight of areas of responsibility, and to ensure consistency for all Department staff. The role includes overseeing the continuous assessment and improvement of the care coordination department and reporting pertinent information regarding patient problems, personnel problems, progress re: projects and initiatives, and any unusual occurrences to the Vice President.
