About The Position

The Center for Health Equity & Community Wellness (CHECW) focuses on social determinants of health, including environmental and commercial determinants, to improve the health and well-being of New Yorkers. The Bureau of Equitable Health Systems (BEHS) is the healthcare bureau of DOHMH, aiming to improve equity in healthcare delivery through policy, evidence, and practical expertise. They engage healthcare organizations to implement strategies, leverage information for planning and technical assistance, advance policy to close racial equity gaps, and identify opportunities for healthcare to connect consumers to social support. The Bureau of Equitable Health Systems seeks to hire a Director, Care Transformation to lead system-wide care transformation and interoperability efforts, focusing on optimizing health information exchange and care coordination across clinical sites.

Requirements

  • A baccalaureate degree from an accredited college and five years of full-time professional experience in a health services setting (e.g., laboratory, hospital, patient care facility, public health, environmental health, or mental hygiene program), including at least 18 months in a managerial capacity.
  • Managerial experience must demonstrate the ability to perform complex program management, resource allocation, and independent decision making.
  • Demonstrated experience supporting Patient-Centered Medical Home (PCMH) transformation, recognition, or renewal activities.
  • Experience serving as a practice facilitator, quality improvement advisor, technical assistance provider, or program manager supporting primary care practices.
  • Knowledge of NCQA Patient-Centered Medical Home standards, quality measurement, clinical workflow redesign, care coordination, population health management, and performance improvement methodologies.
  • Experience leading or supporting value-based payment initiatives, alternative payment models, quality incentive programs, or health care delivery system transformation efforts.
  • Experience working with federally qualified health centers (FQHCs), hospital-affiliated ambulatory care networks, independent primary care practices, or community-based provider organizations.
  • Familiarity with quality improvement methodologies such as Plan-Do-Study-Act (PDSA), Lean, or Six Sigma.
  • Experience managing contracts, grants, technical assistance programs, or provider engagement initiatives in a public health or health care setting.
  • Experience analyzing quality, utilization, operational, or population health data to inform program planning and performance improvement efforts.
  • Proficiency in Microsoft Office Suite; experience with CRM systems (e.g., Salesforce) preferred.
  • A baccalaureate degree from an accredited college and five years of full-time professional satisfactory experience acquired within the last fifteen years, in a health services setting such as a laboratory, hospital, or other patient care facility, or in a public health, environmental health, or mental hygiene program, at least 18 months of which must have been in a managerial capacity, consisting of managerial experience clearly demonstrating the ability to perform difficult and responsible managerial work, requiring independent decision-making concerning program management, planning, allocation of resources, and the scheduling and assignment of work.
  • Education and/or experience equivalent to the above.
  • Education may be substituted for experience on the basis that each 30 graduate semester credits from an accredited college in hospital administration, public health, public administration, business administration, management or administration can be substituted for one year of non-managerial experience up to a maximum of 60 semester credits for two years.
  • However, all candidates must have a minimum of a baccalaureate degree and 18 months of managerial experience as described above.

  • Possession of a Patient-Centered Medical Home Certified Content Expert (PCMH CCE) credential is strongly preferred.

Responsibilities

  • Plan, develop, and administer public health programs, including Data Collection for Equity, Patient Centered Medical Home (PCMH), Health IT and data interoperability, advancing Health-Related Social Needs, and emergency preparedness.
  • Oversee the establishment, implementation, and monitoring of new programs, ensuring alignment with public health goals.
  • Administer contracts, allocate funds, and ensure compliance with program deliverables.
  • Evaluate program performance, identifying successes, barriers, and areas for improvement.
  • Develop systems to track and analyze demographics, trends, and external activities with large primary care practices.
  • Lead strategic discussions and contribute to the development of new programs and funding applications.
  • Collaborate with interagency stakeholders to align efforts in engaging healthcare delivery sites.
  • Partner with the Evaluation team to assess program impact and effectiveness, disseminating findings through reports and presentations.
  • Provide administrative supervision to professional, clerical, and support staff, ensuring high quality service delivery.
  • Mentor and coach team members, fostering professional development and ensuring activities meet defined goals.
  • Collaborate with NYC REACH directors to implement consistent training plans for field teams, ensuring subject matter expertise and skills remain current.
  • Build and strengthen partnerships with large primary care practices, including Federally Qualified Health Centers (FQHCs), Look-Alikes, multi-site practices, and hospital-affiliated ambulatory centers.
  • Lead engagement with data interoperability organizations, including Health Information Exchanges, to advance health information exchange goals.
  • Collaborate with other DOHMH stakeholders to align efforts in engaging healthcare delivery sites.
