About The Position

Living Resources is a non-profit agency dedicated to empowering individuals with disabilities or other life-altering conditions to live with dignity, independence, and purpose. The Day Community Opportunity Program in Hoosick Falls, Rensselaer County, seeks caring and energetic Direct Support Professionals to support individuals who are independent within the community. The program encourages people to pursue personal goals, build skills, volunteer, participate in activities, and become more involved in their community. Employees are expected to be respectful and supportive of families and work with them as partners.

Requirements

Be at least 18 years of age.
Have a valid New York State driver's license to transport individuals to community activities and appointments.
Have a high school diploma or GED.
Be able to perform all tasks requiring lifting, repositioning, and transferring without restriction.
Be passionate about making a difference.

Responsibilities

Provide direct support and guidance to people supported in developing daily living, social, and independent living skills through individualized habilitation plans and community-based activities.
Document progress, maintain confidential records, and communicate regularly with families, coworkers, and program coordinators.
Provide safe transportation and assist people supported with mobility needs.
Maintain assigned agency vehicles, complete required inspections and reports, follow safety and incident procedures, and remain compliant with required training.
Participate in all agency and departmental trainings and meetings.
