About The Position

Requirements

  • High school diploma or GED.
  • 18 years of age or older.
  • Valid driver’s license in good standing; ability to obtain/maintain a driver’s license required.
  • Successful clearance of background checks and ability to obtain a Level 1 Arizona Fingerprint Clearance Card.
  • Proficient in written and verbal English; able to follow written and oral instructions.
  • Ability to frequently lift/move up to 10 lbs and occasionally lift/move over 100 lbs.
  • Ability to complete required training and pass associated tests.
  • Mandatory reporter responsibilities: report suspected abuse/neglect to appropriate authorities (DCS/APS) and supervisor.

Nice To Haves

  • Six months of human services experience preferred.
  • Current CPR & First Aid certifications (or ability to obtain).

Responsibilities

  • Collaborate with medical teams and case managers to deliver person‑centered care.
  • Assist members with daily living tasks and accompany them to appointments as needed.
  • Provide assistance, training, and monitoring in self‑care, independence, mobility, and learning per the Individual Service Plan (ISP) to promote skill development, health, autonomy, and dignity.
  • Maintain a clean, safe home environment.
  • Communicate effectively with clients, families, and team members.
  • Support behavior management and crisis intervention when necessary.
  • Perform health‑related tasks including medication administration, meal planning, allergy management, and basic first aid.
  • Teach and reinforce safety procedures, participate in fire drills, and respond to emergencies.
  • Maintain confidentiality and comply with federal, state, and local regulations.
  • Complete and maintain accurate documentation and incident reports.
  • Maintain reliable attendance and follow time‑tracking policies.
  • Perform additional duties as assigned.
