Support individuals with all of their needs, ensuring their health and safety. Assist them with vocational tasks, acting as a mentor and example to all.

Requirements

  • GED/High School diploma
  • 3 years’ experience in a DSO setting
  • Must be reliable, dependable, and flexible with hours
  • Must possess proven experience and success
  • Must possess valid driver’s license
  • Must have 5 years of clean driving experience
  • Must be able to work as part of a team as well as be independent

Nice To Haves

  • CPR/First Aid certification
  • Medication certification
  • PMT certification

Responsibilities

  • Provide training, supervision, instruction, and support to individuals in all agency settings, on site or in the community.
  • Ensure awareness of assigned individuals' whereabouts during the day, communicating with other staff when leaving the floor and ensuring responsibility is transferred.
  • Attend required trainings and meetings, providing input on individual plan development.
  • Assist in developing and tracking worker goals and other daily documentation.
  • Be aware of all Mealtime Procedures and supervise individuals during meals.
  • Provide transportation as needed for outings.
  • Provide ongoing support as indicated by individual plans (IPs), assisting individuals to complete tasks as independently as possible.
  • Assist with securing community-based activities/work sites for individuals.
  • Assist staff and individuals during fire drills or other emergency situations.
  • Pass medications if applicable.
  • Assist in providing a positive program environment for staff and participants.
  • Complete and maintain all daily individual documentation and worker weekly notes.
  • At the end of each month, remove all data collection sheets and weekly notes from assigned individuals' books.
  • Ensure completion of all entries and turn them into the Assistant Coordinator each month.
  • Ensure all extra documents are filled out by the end of the day if applicable (e.g., 255, Seizure protocol, behavior data, ABC's, minor incidents).
  • At the beginning of each month, ensure new data collection pages and weekly notes are placed in assigned individuals' books.
  • Sign off on completed Paylocity punches and submit with any mileage forms to the Assistant Coordinator by Friday PM.
  • Submit Auto Insurance Declaration page to Human Resources upon renewal or every 6 months.
  • Document appropriately in all Travel Books when using Company Vehicles.
  • Inform immediate supervisor if any trainings are nearing expiration, providing 60 days' notice.
  • Have knowledge of and follow all participants' water safety, fall risks, behavior, toileting, and LON requirements.
  • Provide assistance to peers as needed.
  • Be a team player.
  • Learn and follow all agency policies and procedures (P&P).
  • Learn and follow all emergency and safety procedures for the agency.
  • Be involved in OWC fundraising efforts.
  • Perform other duties as they arise.
  • Maintain mutual respect and professionalism for all co-workers and individuals daily.
