About The Position

Easterseals PORT Health is seeking a Direct Support Professional for its group home in Zebulon, North Carolina. The role supports people in residential settings by participating in daily home routines and providing habilitative and rehabilitative activities, personal care, health care, and other services described in each person’s care or support plan. Services follow person-centered and informed-choice philosophies.

The Direct Support Professional helps individuals with daily living, personal care, home care, and transportation, and supports the development of socialization, behavior management, daily living, and community skills. The position is offered full-time or part-time, with several schedule options, and pays $15 per hour.

Requirements

• High school diploma or equivalent.
• Valid driver’s license and auto insurance.
• Ability to lift at least 50 pounds, bend, and assist individuals with mobility needs.
• Ability to speak audibly, write legibly, and perform repetitive motions.

Nice To Haves

• One year of documented experience with the population served is preferred.

Responsibilities

• Provide person-centered support with activities of daily living, personal care, household tasks, transportation, medication management, and community engagement.
• Partner with individuals, families, supervisors, healthcare providers, and community resources to support person-centered goals, maintain safety, and foster community inclusion.
• Coordinate and deliver direct support services, including skill-building interventions, behavior support, meal preparation, medication assistance, recreational activities, and implementation of individualized support plans.
• Document and maintain progress notes, medication administration records, incident reports, health and safety observations, and other required records.
• Collaborate with team members, participate in supervision and training, support quality improvement initiatives, and demonstrate accountability, commitment, and caring.
