About The Position

We are looking for someone who is compassionate with a person-centered mindset, has a warm, respectful, and professional demeanor, and is capable of communication with dignity and respect, especially during challenging moments. A valid driver’s license and reliable form of transportation are required. Bilingual candidates are encouraged to apply. Experience with participants who smoke and professional experience with behaviors is a plus. The role requires someone responsible who can document goal progress tracking with detail. DSP 1 & 2 certification and CPR/First Aid certification are preferred, but training will be provided if applicants are not certified or need to be recertified. Weekend availability is a plus but not required.
Requirements

Responsibilities
- Support participants with daily living skills such as cleaning, cooking, budget management, and light household duties
- Following care plans and helping participants achieve their personal goals
- Encourage healthy habits, studies, hobbies, and social activities
- Assist participants access and participate in community activities
- Assist with personal care such as medication support, hygiene and bathing (lifting / transferring may be required)
- Collaborate Person-Centered Care planning with participant, staff and management
- Document shift and communication logs
