Requirements

  • Valid Driver's License
  • Access to a vehicle in good operating order.
  • High school diploma or equivalent
  • Experience working with individuals with developmental disabilities with behavioral needs and mental health conditions

Responsibilities

  • Helps plan and implement community interactive experiences and interest-based classes as specified in each client's individual support plan (ISP).
  • Provide communication to supervisors regarding individuals served, about their home, day program, or work related needs as applicable to each individual.
  • Completes documentation as required by state and federal regulations.
  • Complies with behavior support policies and procedures and implements approved behavior support programs.
  • Maintain employment-required documentation including but not limited to first aid, CPR, Medication Administration, and Relational/Technical MANDT, as required to comply with federal, state, and program requirements and standards.
  • Implement all protocols for individuals including, but not limited to: high risk plans, behavior support plans, dining plans, seizure management plans, positioning schedules, fall risk plans, etc, as applicable to each individual.
  • Complete proper documentation relating to the healthcare of the individuals served, including MARS, healthcare notes, etc.
  • Attends departmental meetings and completes a monthly activity schedule based on the recommendations of the team meeting as well as the needs/wants of the individuals served as outlined in their person-centered plans.
  • Attends all required training and provides HR with updated personal information as mandated.
