About The Position

The Direct Support Professional provides training and support in Activities of Daily Living to individuals residing in their own homes and implements the goals and valued outcomes identified in the Individual Service Plan. This role operates within a day habilitation program environment and requires the ability to assist with personal care, lift over 75 pounds, and potentially respond to behavior interventions. The position involves community integration and transportation of individuals served.

Requirements

  • High School Diploma or equivalent required.
  • Valid New York State driver’s license with a clean driving record required.
  • Trained in CPR/First Aid and maintained according to Red Cross standards.
  • Clearance from Office of Children and Family Services (OCFS) which includes NYS Department of Criminal Justice Services, Federal Bureau of Investigation, Statewide Central Register Database Check and NYS Justice Center.
  • Ability to communicate effectively with individuals, families, staff, management and administration.
  • Will successfully complete required training courses.
  • Multicultural awareness and respect for all people in accordance with agency standards, professional conduct and ethics.
  • The employee displays understanding of the roles and responsibilities of all other staff, demonstrating effective problem solving and decision-making abilities in collaboration with the other team members and administration.
  • Ability to assist with some personal care/toileting.
  • Ability to lift over 75 pounds.
  • May need to respond to a behavior intervention with an individual in the means taught to them in Strategies for Crisis Intervention and Prevention.
  • May be required to travel to various job sites or locations as part of working with individuals.
  • Ability to react quickly, which may include but is not limited to stooping, bending, sitting, standing, stretching, walking and running.

Responsibilities

  • Provide training, onsite and in the community, according to consumer needs, wants and desires as outlined in the Life Plan.
  • Display mature judgment in all areas including maintaining confidentiality and handling emergencies.
  • Act as a professional representative of the E. John Gavras Center.
  • Responsible for the provision of on-going assistance, skills training, supervision and support in the Activities of Daily Living including but not limited to: Health skills and monitoring of medication and medical issues; Personal Hygiene/Personal Care needs; Domestic living skills; Money management; Menu Planning, food procurement and meal preparation; Socialization and community integration; Recreation and leisure time activities; Safety education and training; Transportation skills training; Behavioral interventions.
  • Responsible for following all Agency policies and procedures as well as OPWDD Regulations.
  • Maintain professional boundaries while working with the Individuals' diagnosed with Developmental Disabilities.
  • Empower, Motivate, and support Individuals in every aspect of daily living skill while promoting independence.
  • Responsible for the well-being of all people in the program with primary duties assigned by the supervisor or designee.
  • Exhibits sensitivity to and respects the rights of the individuals served, including their emotional and physical needs.
  • Communicates needs or desires of the individual in both verbal and written form via protocols established by the program.
  • Provides training, supervision, assistance and monitoring in areas related to the acquisition and retention of daily living skills with the ultimate goal of independence for the people we serve using established life plans.
  • Facilitates community integration and inclusion by locating activities of interest to individuals served while exploring with individual's alternative avenues of interest.
  • Assists in the development of the individual's plans including documentation and reporting data and other paperwork as required by agency or program.
  • Transports individuals served in agency vehicles as needed, requested, or scheduled.
  • Adheres to all agency, local, state and federal transportation guidelines to ensure safety of individuals being transported.
  • Demonstrates flexibility in order to meet the needs of individuals served, demonstrating the ability to creatively and proactively address new situations, showing initiative and caring attitude towards individuals served, other staff and outside contacts.
  • Obtain and maintain all required certification within the required timeframe including First Aid, CPR, SCIP, and Exposure Control.
  • Participate as a team in frequent meetings to discuss consumer programming, training issues and methods for implementation of individualized consumer goals.
  • Maintain appropriate documentation according to agency policies, completing daily notes and reports in a timely fashion.
