About The Position

The Center for Human Development (CHD) is seeking Direct Care Staff to join their team in Hartford, CT at their CTLP (Community Transitional Living Program) program. This role involves establishing life-changing relationships with participants and providing opportunities to optimize their independence, rehabilitation, and recovery. CHD’s Community Transitional Living Program (CTLP) is a community-based transitional independent living environment for up to 19 eligible adults with severe and persistent mental health diagnoses who will benefit from temporary residential support. Residents benefit from 24/7 staff monitoring and support while living in their own apartments within a single building, and working to achieve their goals, including discharging to even more independent living. Residents receive support in ADL’s, skill building, life skills, social/recreational activities, self-care, household tasks, education/employment, transportation, finances, and much more.

Requirements

  • Experience working with psychiatric disorders or co-occurring population preferred
  • High school diploma or GED
  • Valid driver's license
  • Reliable, registered & insured vehicle and approved driving record check
  • All candidates must complete and pass background record screening process
  • Current First Aid, CPR and MAP certification preferred

Responsibilities

  • Works within the Agency’s team model, establishing open communication, support, and accountability among co-workers.
  • Providing a supportive residential environment for individuals who are challenged by mental health and substance abuse histories in a community-based group home.
  • Providing direct services to program clients living in a group home setting in the community.
  • Supporting clients' activities of daily living and monitoring clients' self-administer medication.
  • Advocating for individuals' within various community settings, and providing opportunities to optimize their independence, rehabilitation and recovery.
  • Develop professional and supportive relationships with clients and provide informal counseling and guidance with a person-centered focus.
