About The Position

The Center for Human Development (CHD) is seeking Direct Care Staff to join their team at the Iris residential program in New Hartford, CT. This role involves establishing life-changing relationships with participants and providing opportunities to optimize their independence, rehabilitation, and recovery. The Iris group home program serves individuals with high levels of chronic psychiatric conditions, aiming to provide them with a high quality of life. The program specializes in supporting clients who have faced challenges with community living by meeting individual needs through unique treatment plans. To ensure a high level of care, Iris House provides 24/7 on-site staffing and a team-based approach to client care. The goal is to provide structure, stability, care, and attention to help clients overcome behaviors learned from self-preservation and reveal their true selves.

Requirements

High school diploma or GED
Valid driver's license
Registered & insured vehicle available for work purposes
Approved criminal background and driving check

Responsibilities

Works within the Agency’s team model, establishing open communication, support, and accountability among co-workers.
Provides a supportive residential environment for individuals challenged by mental health and substance abuse histories in a community-based group home.
Encourages program participants to grow and live as independently as possible.
Provides direct services to program clients living in a group home setting in the community.
Supports clients' activities of daily living and monitors clients' self-administration of medication.
Advocates for individuals within various community settings.
Develops professional and supportive relationships with clients.
Provides informal counseling and guidance with a person-centered focus.
