Direct Care Staff - New Hartford
About The Position
The Center for Human Development (CHD) is seeking Direct Care Staff to join our team at our Iris residential program in New Hartford, CT. Our direct care staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation and recovery. Our Iris group home program is dedicated to servicing individuals with a high level of chronic psychiatric conditions, enabling them to live a life of high quality. The program specializes in supporting clients who have experienced challenges with community living by ensuring the individual needs are met through each client’s uniquely created treatment plan. To ensure a high level of care, Iris House supports clients by providing 24/7 on-site staffing. Iris House takes a team-based approach to the all-around care of the clients. While clients may initially struggle to shed behaviors learned from years of self-preservation, we provide the structure, stability, care and attention needed to let go of their diagnoses to reveal the person underneath.
Requirements
- High school diploma or GED
- Valid driver's license
- Registered & insured vehicle available for work purposes
- Approved criminal background and driving check
Nice To Haves
- Experience working with psychiatric disorders or co-occurring population preferred
Responsibilities
- Works within the Agency’s team model, establishing open communication, support, and accountability among co-workers.
- Providing a supportive residential environment for individuals who are challenged by mental health and substance abuse histories in a community-based group home.
- Providing an environment that encourages program participants to grow and to live as independently as possible.
- Providing direct services to program clients living in a group home setting in the community.
- Supporting clients' activities of daily living and monitoring clients' self-administer medication.
- Advocating for individuals' within various community settings, and providing opportunities to optimize their independence, rehabilitation and recovery.
- Develop professional and supportive relationships with clients and provide informal counseling and guidance with a person-centered focus.
