JOB SUMMARY: The registered dietitian role functions to provide advice and information on food and nutrition. A registered dietitian may have to assess the needs of patients, provide instructions/education based on an individual’s goals/needs, and develop meal plans and follow up with the effectiveness of those plans for patients.

Requirements

  • Registered Dietitian and Certified Diabetes Care and Education Specialist
  • English proficiency required
  • Valid license based on location of practice as a Dietitian

Responsibilities

  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops and individualized care plan based on the patient’s assessed needs and goals and promotes successful self-management.
  • Provides instruction/education on appropriate diet, individualizing goals based on the needs and abilities of each patient.
  • Describes the differences in disease risk factors, diagnosis, treatments, and progression of diabetes and cardiometabolic conditions across the life span.
  • Explains physiology of glucose metabolism and pathophysiologic mechanisms responsible for the development of diabetes and cardiometabolic conditions.
  • Evaluates appropriateness of medications for diabetes and evaluates medications, over-the-counter agents and dietary supplements for potential interactions.
  • Assist individuals with recognition of barriers and codevelops strategies for uninterrupted medication use.
  • Explains the relationship between food, activity, medication and clinical outcomes, such as blood glucose, lipids, blood pressure and weight.
  • Applies knowledge of technology and person’s interest in and comfort with technology to select most appropriate tools.
  • Explains available tools used for monitoring, such as self-monitoring of blood glucose, continuous glucose monitoring, and mobile applications appropriate techniques, for accurate collection of patient-generated health data.
  • Documents all individual contacts/visits in the electronic health record and outcomes data base according to the guidelines in a timely manner.
  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, and self-management of nutrition-related health conditions into clinical practice.
  • Collaborates with community partners to establish and maintain on-going support options.
  • Performs other work-related duties as assigned.
  • This position is required to participate in mandatory all staff meetings, team meetings and trainings.
