About The Position

Explores, recommends, and coordinates the varied community resources and social services programs available to kidney dialysis patients. This role is responsible for driving company culture through values and customer service standards, developing and maintaining effective relationships, and taking initiative to resolve customer service issues. The position involves collaborating with local offices to obtain insurance assistance, coordinating pharmacy and prescription programs, and navigating social services and community resources such as emergency food, housing, and shelter. It also includes assisting with transportation referrals, authorizations, and re-certifications, identifying appropriate community resources for patients and families, and ensuring compliance with company policies and laws. Additional responsibilities may focus on specific departments or locations.

Requirements

  • Bachelor's Degree in Social Work (BSW) required.
  • 0 - 2 years of Healthcare experience or related with specific knowledge of and experience with navigating community resources.
  • Strong oral and written communication skills.
  • Detail oriented.
  • Strong organizational and time management skills.
  • Proficient with PCs and Microsoft Office applications.

Responsibilities

  • Driving the company culture through values and customer service standards.
  • Developing and maintaining effective relationships through effective and timely communication.
  • Taking initiative and action to respond, resolve, and follow up regarding customer service issues.
  • Collaborating with local offices to obtain Medicare and Medicaid Insurance Assistance.
  • Coordinating Pharmacy Program benefits and prescription assistance programs.
  • Social service and community resource navigation and referral, including emergency food, housing, and shelter resources.
  • Assisting with transportation referrals, authorizations, and re-certifications.
  • Identifying appropriate community social service agencies and other resources to assist patients and families.
  • Assisting patients in securing financial assistance, obtaining medical transportation, and any other public and private resources deemed necessary.
  • Reviewing and complying with the Code of Business Conduct and all applicable company policies and procedures, local, state, and federal laws and regulations.
  • Assisting with various projects as assigned by direct supervisor.
  • Performing other duties as assigned.
  • Floating to Kamuela 1-2x/week.
