Dialysis Home Therapies Nurse (RN)
About The Position
Dialysis Clinic, Inc. is seeking a compassionate registered nurse to support its nonprofit mission of individualized care for patients facing chronic kidney disease. The Dialysis Home Therapies Nurse coordinates the administrative and clinical aspects of the home dialysis program, including home hemodialysis and peritoneal dialysis, and communicates with patients and support staff to maintain high standards of care. The schedule is Monday through Friday, with on-call responsibilities as needed. After training, the work location will mainly be in the Freehold/Monroe area, with occasional days at the New Brunswick clinic. The hourly pay range is $42–$57, depending on nursing and dialysis experience. Paid training is offered.
Requirements
- Accredited nursing degree.
- Current New Jersey RN license.
- Valid driver's license and safe driving record.
- Excellent communication skills and a desire to teach.
- Demonstrated clinical excellence.

- Ability to problem solve.

Responsibilities
- Collaborate with the medical team on patient care concerns, hospitalizations, discharges, and modality changes.
- Coordinate home training sessions, clinic visits, patient care conferences, home visits, and clinical and educational activities to support efficient use of nursing hours and program facilities.
- Monitor patient supply ordering and usage while maintaining quality care.
- Communicate regularly with the interdisciplinary care team about home program activities.
- Monitor ongoing patient care parameters.
- Perform registered nurse functions and duties.
- Create and maintain records, patient training documentation, and other documentation in accordance with ESRD Network requirements, regulatory agencies, DCI's CQI program, and facility policies and procedures.
