About The Position
DaVita is seeking a Patient Pathways Liaison to facilitate the seamless transition of patients from Alta Bates Summit Medical Center in Oakland and Berkeley hospital settings to our outpatient dialysis care. This role is ideal for a coordinator or clinical professional who excels at insurance verification, hospital collaboration, and patient advocacy. You’ll empower patients facing kidney disease by providing vital education and ensuring they have a seamless transition to the dialysis center of their choice.
Requirements
- Minimum of 2 years of recent healthcare experience is preferred.
- Dialysis experience is a huge plus and highly preferred!
- Vocational Certificate or Associate’s Degree.
- Proficiency in insurance verification, navigating healthcare systems, and high-volume coordination.
- A passion for clinical education and a desire to help patients make informed healthcare decisions.
Responsibilities
- Educate & Advocate: Meet with patients to discuss dialysis options, insurance, and treatment modalities with empathy and clarity.
- Facilitate Placements: Coordinate with hospital teams to manage referrals and ensure all medical documentation is ready for a smooth transition.
- Build Partnerships: Strengthen relationships with local hospitals and doctors to improve the patient experience across the Bay Area.
