DaVita Kidney Care is launching a new team, DaVita Skilled Nursing Dialysis Services (DSD), to provide dialysis care on-site at skilled nursing facilities (SNFs). This initiative aims to reduce the burden on patients who are transported for dialysis and lower healthcare costs. DSD teammates will leverage DaVita's resources to grow this new service line. The ideal candidate is experienced within DaVita, thrives in a start-up environment, is passionate about improving patient lives, and can work autonomously while collaborating with cross-functional teams. In this role, you will be responsible for the complete ongoing care of each patient to ensure their dialysis treatment stays on track, working in conjunction with dialysis RNs. You will work three days a week with no required on-call, but your impact will be significant. DSD Dialysis care is complex, requiring multitasking to meet the needs of the service, patients, and SNF partners, with a focus on meeting and exceeding clinical outcomes. You will provide comprehensive patient care in an independent setting, constantly using critical thinking skills to monitor patients before, during, and after dialysis treatment, including measuring stats, patient observations, and hemodialysis machine setup. Training will include classroom and hands-on learning through DaVita's award-winning programs, potentially at a training clinic or assigned home clinic. Hours may vary during training, typically 32-40 hours per week. You may also be asked to support other local SNF dens.
