About The Position

The Diagnostic Assessor evaluates current client and family functioning, explains mental health services offered by OhioGuidestone, and engages families, youth, and other pertinent agencies. This role involves completing mental health diagnostic assessments and formulating diagnoses according to the DSM-5 criteria, while maintaining current knowledge of these diagnoses and recommending appropriate treatment options and levels of care. The position also includes administrative responsibilities such as reviewing and completing reports and records within required timeframes, responding to communications promptly, attending trainings and meetings, and collaborating with other agencies and departments. The assessor will obtain necessary documents, information, and signatures from parents and clients to complete assessments and evaluations, and will summarize major treatment issues and expected outcomes, recommending case disposition to OhioGuidestone programs. Client meetings can occur virtually, at OhioGuidestone locations, or in client homes. The role requires maintaining a minimum productivity level, ensuring client confidentiality, and providing at least three available times weekly for diagnostic evaluation appointments. The position also emphasizes exhibiting positivity, flexibility, a willingness to take on new responsibilities, positive leadership, promoting a team-based work environment, and presenting the Agency positively.

Requirements

  • Master's degree and Ohio LSW or LPC required.
  • Experience with Electronic Health Records (EHR), Evolv preferred.
  • Ability to customarily and regularly exercise discretion and good professional judgment.
  • Ability to maintain relevant, timely documentation as required.
  • Effective computer literacy skills, including Windows applications, Microsoft Office Suite, and electronic health records, etc.
  • Must be able to access work locations, offices, and homes as necessary, which may include ascending/descending stairs.
  • Possess a valid Ohio Driver’s License with a safe driving record and valid insurance.
  • Successfully complete pre-employment screenings, including drug tests, background checks, and any other required assessments, such as a physical examination and TB test.

Responsibilities

  • Evaluates current client and family functioning.
  • Explains the mental health services of OhioGuidestone.
  • Engages family, youth, and other pertinent agencies.
  • Completes mental health diagnostic assessments and formulates a diagnosis according to the DSM-5 criteria.
  • Recommends treatment options and level of care.
  • Performs related administrative responsibilities, including reviewing and completing reports, records, and other documents within the required timeframe.
  • Responds to emails, texts, and phone calls within a 24-hour period.
  • Attends various trainings and meetings.
  • Communicates with other agencies, departments, and vendors regarding required paperwork, contract compliance.
  • Consults with other county agencies and/or supportive services as necessary.
  • Obtains documents, information, and signatures from parents and clients that are necessary to complete assessments and evaluations.
  • Summarizes the major treatment issues and expected outcomes.
  • Recommends case disposition to OhioGuidestone programs.
  • Meets with clients virtually, at all OhioGuidestone locations, or travels to client’s homes to complete assessments.
  • Maintains minimal productivity level and the confidentiality of client or collateral interaction within Agency defined standards.
  • Provides a minimum of three available times weekly for diagnostic evaluation appointments.
  • Exhibits positivity, flexibility and a willingness to take on new responsibilities as requested or required.
  • Demonstrates positive leadership, promotes a team-based work environment and a culture of belonging.
  • Presents the Agency in the most positive light with all internal and external contacts.
