The Diabetic Educator provides care following the National Standard for Diabetes Self-Management Education and Support (DSMES) as defined by the American Diabetes Association (ADA) or Association of Diabetes Care and Education Specialist (ADCES). This role involves assessing patient learning needs, goals, and barriers to care, and tailoring educational interventions accordingly. The educator will lead individual and group education sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring. If a Registered Dietitian, they will provide medical nutrition therapy services for patients with diabetes or other referring diagnoses. The position also includes providing ongoing support and follow-up to ensure successful management of diabetes or other referring diagnoses, maintaining knowledge of diabetes management technology and manufacturer protocols, and collaborating with the healthcare team to develop and implement comprehensive care plans. The educator will stay current on developments in diabetes care and education, ensuring compliance with ADA standards, and document and charge for services provided consistent with CMS and payor requirements. This role requires the ability to demonstrate knowledge and skills appropriate for the age of the patients served and an understanding of the principles of growth and development over the life span. The educator will also serve as a health resource to the community by participating in community events and coordinating support meetings, and will participate in the development and evaluation of patient education program and materials, acting as a resource for health care providers, patients, and the community. If a teammate does not yet have CDCES, they must complete the required hours of diabetes-related continuing education annually, per ADA /ADCES requirements.
