Requirements

• Bachelor's Degree in nursing from an accredited institution and 3 years of professional nursing experience Required OR Bachelor's Degree with major coursework in human nutrition, food and nutrition, dietetics, or food systems management from an accredited institution, and 3 years of professional dietitian experience Required
• RN - Registered Nurse - State Licensure and/or Compact State Licensure Licensure with the state of Alabama as a registered nurse Required or REG DIET - Dietitian – Registered with the Commission on Dietetic Registration Required and Licensure with the state of Alabama as a dietitian. Required
• Advanced knowledge of diabetes pathophysiology, treatment, self-management, and prevention of acute and chronic complications.
• Knowledge of current diabetes medications, insulin therapies, injectable medications, glucose monitoring systems, CGM, and other diabetes technologies.
• Knowledge of evidence-based DSMES principles and person-centered education practices.
• Knowledge of ADA Standards of Care and the National Standards for DSMES.
• Strong patient assessment, clinical judgment, problem-solving, and critical-thinking skills.
• Ability to independently provide diabetes education while recognizing issues requiring consultation with or escalation to a prescribing provider.
• Ability to design, deliver, and evaluate individual and group educational programs.
• Strong classroom facilitation and presentation skills.
• Ability to interpret patient glucose data and translate clinical information into practical self-management education.
• Excellent written, verbal, interpersonal, and interdisciplinary communication skills.
• Strong organizational skills and attention to documentation, regulatory, and program requirements.
• Ability to use electronic health records, Microsoft Office applications, diabetes technology platforms, and program data systems.
• Ability to work effectively with diverse patient populations, caregivers, healthcare professionals, and community partners.

Nice To Haves

• Master's Degree in nursing and experience in diabetes care and chronic disease program management Preferred
• Certified Diabetes Consult and Education Specialist (CDCES) within 1-1/2 Yrs Preferred
• Comparable combination of education and experience may substitute for the above requirements.

Responsibilities

• Assesses patient learning needs and develops individualized education plans and self-management goals.
• Provides education on diabetes management, medications and insulin, glucose monitoring and diabetes technology, nutrition, physical activity, risk reduction, problem solving, and healthy coping.
• Reviews relevant clinical information, glucose data, CGM reports, and treatment regimens to identify educational and self-management needs.
• Provides appropriate follow-up education and support based on patient needs and provider referrals.
• Provides diabetes education consultations as patient needs arise and serves as a clinical resource for patients, caregivers, and healthcare professionals.
• Documents assessments, education, interventions, goals, and follow-up plans accurately and promptly in the electronic health record.
• Collects and maintains required participant and program outcome data, including documentation in Chronicle Pro.
• Ensures documentation meets organizational, payer, regulatory, and ADA Recognition requirements.
• Maintains program documentation and assists with annual reviews, ADA reporting and Recognition requirements.
• Participates in continuous quality improvement activities and evaluates program and participant outcomes.
• Assists with development and maintenance of DSMES policies, procedures, curriculum, educational materials, and workflows.
• Assists with staff in-services, continuing education, and diabetes-related competency activities.
• Collaborates with departments, providers, and community organizations to promote access to quality diabetes care and education.
• Participates in relevant committees, meetings, outreach activities, and professional development leading to certification.
• Completes all mandatory department, educational and hospital requirements
• Adheres to current Infection Control and Safety Standards
• Regular and prompt attendance
• Ability to work schedule as defined and overtime as required
• Related duties as assigned
