About The Position

The Registered Dietitian/Diabetes Educator functions as a registered dietitian/diabetes educator in a variety of settings, including inpatient Diabetes Education, the Outpatient Diabetes and Nutrition Self-Management Education program, community events and education, and the Diabetes Support Group. Primary responsibilities include screening, assessing, developing, and implementing individualized nutritional care and education as well as diabetes self-management education to individuals, families, and groups and other patient populations serviced; collaborating with the interdisciplinary health care team to facilitate continuity of care at home and in the community; and maintaining professional competency and skills required for professional practice.

Requirements

  • A Bachelor degree in Nutrition or Dietetics required
  • Registered Dietitian by the Commission on Dietetic Registration of the Association of Nutrition and Dietetics required
  • Diabetes Care and Education Specialist (CDCES) Certification OR Eligible and willing to obtain required CDCES or BC-ADM certification within two (2) years of DCES hire date including documented completion of 15 CEUs in diabetes-related education within 4 months of hire and each ADA anniversary year thereafter until certification is obtained.
  • Minimum 2 years of clinical experience required.

Nice To Haves

  • Master’s in Dietetics or Nutrition or related field preferred
  • Experience with diabetic patient population preferred.

Responsibilities

  • Provide Medical Nutrition Therapy for diabetes and other chronic diseases to patients and families.
  • Provide diabetes self-management education and training to patients and family members utilizing curriculum approved by the American Diabetes Association.
  • Plan, develop, and implement individual and group diabetes education for patients, their families, and the community.
  • Provide appropriate documentation that summarizes the diabetes self-management care plan in the patient’s medical record.
  • Work as inpatient diabetes educator screening and addressing the identified high-risk patients with diabetes and those patients for whom referrals have been received.
  • Serve as resource to medical staff and other health personnel on medical nutrition therapy and diabetes self-management issues.
  • Train and educate hospital staff in nutrition education and/or diabetes self-management education.
  • Assist in the education of various students and interns (dietetic, PA, and NP). Plan learning experiences, teaches, and evaluates performance.
  • Maintain productivity standards and practices effective time management and prioritization of tasks.
  • Manage resources, including time, materials, and staff, in a cost-effective manner, developing and revising patient education materials, policies and procedures, as needed.
  • Assist in achieving compliance with accrediting organizations and regulatory agency standards.
  • Identify, design, and collect improvement data, monitor outcomes, and initiate corrective actions.
