About The Position

The Diabetes Care & Education Specialist provides education, support, and guidance to individuals with diabetes and their families to promote optimal self-management and improve health outcomes. This role involves assessing patient needs, developing personalized care plans, and collaborating with healthcare teams to ensure comprehensive diabetes care. Requirements

  • Bachelor’s degree in Nursing
  • Knowledge of diabetes pathophysiology, pharmacology, and self-management strategies
  • Strong communication, teaching, and counseling skills
  • Patient-centered approach with empathy and cultural sensitivity
  • Ability to motivate and empower patients toward self-management goals
  • Strong critical thinking and problem-solving skills in complex patient situations
  • Ability to work both independently and collaboratively within a multidisciplinary team
  • Excellent organizational skills with strong attention to detail
  • Proficiency with electronic medical records (EMR) and patient education tools
  • Excellent, visual, hearing, speech/articulation abilities.
  • Manual dexterity to use computer to communicate, investigate and educate.
  • Ability to sit at a computer for long periods of time (2-3 hours)

Nice To Haves

  • Certification in Diabetes Education (CDCES) preferred or willingness to obtain

Responsibilities

  • Conduct assessments of patients’ knowledge, skills, and readiness for diabetes self-management.
  • Develop and deliver individualized diabetes education covering topics such as nutrition, blood glucose monitoring, medication management, diabetes-related technologies, lifestyle modification, and complication prevention.
  • Provide ongoing support, counseling, and follow-up to encourage adherence and behavioral changes.
  • Monitor patient progress and adjust education plans as needed.
  • Coordinate centralized continuous glucose monitoring (CGM) workflows, including device onboarding, data integration, and patient follow-up.
  • Provide remote patient monitoring of CGM data to identify trends, support timely interventions, and communicate recommendations to the care team.
  • Serve as a resource for insulin pump and automated insulin delivery (AID) systems, including patient education, troubleshooting, and therapy optimization.
  • Collaborate with healthcare providers, including physicians, nurses, dietitians, and pharmacists, to ensure coordinated care.
  • Maintain accurate documentation of education sessions and patient outcomes in accordance with regulatory standards.
  • Stay current with advances in diabetes care, treatment guidelines, and patient education strategies.
  • Participate in community outreach, workshops, and support groups as needed.
