About The Position

Under the supervision of the Senior Vice President of Adult Residential and Day Training of Elmwood Heights and East, oversees the development, implementation, monitoring, and delivery of all individual services for assigned homes, ensuring the optimum quality of life and level of independence possible in the least restrictive setting for the individuals of Elmwood Heights/Elmwood East ICF/DD.

Requirements

  • Minimum of two years’ experience as RSD/QDIP including program development and implementation.
  • Demonstrates good oral and written communication skills.
  • Clean driving record and must be approved to drive Milestone vehicles.
  • Must be able to intervene physically in behavioral and health related crisis with individuals.
  • Must be able to promptly respond to any/all emergency situations.
  • Licensed as a Nursing Home Administrator, or eligible for Licensure.

Responsibilities

  • Oversees the development, implementation, monitoring, and delivery of all individual services for assigned homes.
  • Ensures the optimum quality of life and level of independence possible in the least restrictive setting for the individuals.
  • Prepares documents required for licensure of the facility.
  • Directs and supervises work performance of assigned Director of Nursing, Resident Services Director/QDIP and Elmwood Heights Activity Leader including hiring, performance evaluations, disciplinary action, and recommendations for promotion and/or transfer.
  • Provides direction and support to Director of Nursing, Resident Services Directors, QDIP, and Day Training Instructor in orienting, training, and monitoring Team Leaders and Team Members.
  • Develops, implements, and evaluates formalized Leadership Training Programs.
  • Works as a Team with other supervisory Staff, sharing information, working on common issues, demonstrating positive problem-solving skills, and actively providing support and assistance.
  • Oversees self-scheduling of all Elmwood Heights/Elmwood East Technicians, ensuring adequate coverage to meet individual needs and maintain budgetary constraints.
  • Provides input and makes recommendations for annual facility staffing plans.
  • Regularly schedules and conducts inter-departmental meetings to ensure coordination of all services, promoting positive problem solving and effective team functioning.
  • Participates as a trainer in Habilitation Technician Training as assigned, including proposals for format revisions and procedures for completion and documentation of follow-up training in the home.
  • Oversees use of program funds and individual personal funds as directed ensuring responsible fiscal management, acceptable documentation, and maintenance of budgetary constraints.
  • Actively participates as a member of the Interdisciplinary Team in the evaluation of individuals for admission and discharge.
  • Ensures all required documentation is complete and accurate in preparation for their movement.
  • Establishes, maintains, and oversees effective communication between facility personnel and families/guardians, addressing problems as necessary.
  • Intervenes in problematic issues to re-establish an acceptable level of trust, cooperation, and communication between the facility and families/guardians.
  • Regularly inspects assigned homes and ensures the maintenance of an exceptionally clean, safe, and pleasant living environment for the individuals and staff.
  • Ensures the maintenance of a high-quality living environment.
  • Demonstrates knowledge of Illinois Department of Public Aid, Illinois Department of Public Health Standards, and HCFA and ensures assigned homes are in compliance with all authorized inspecting agencies’ standards.
  • Actively and professionally participates in inspections as assigned.
  • Reviews and acts upon reports/complaints of authorized inspecting agencies, including Illinois Department of Public Aid, Illinois Department Health, HCFA, Office of State Guardian, and Human Rights Authority.
  • Meets with other Professional Staff on a regular basis, sharing information, on-going or problematic issues, and developing cooperative solutions.
  • Actively participates in regular IDT/Communication Meetings to ensure coordination of all services and maintains a positive relationship with all departments.
  • Attends staffing’s and reviews assigned written Individual Service Plans, ensuring plans are well written and include all necessary programs and services to meet individuals’ needs.
  • Provide feedback to assigned RSC/QDIP/RSD as necessary to improve quality of written ISPs and ensure adherence to implementation dates.
  • Functions as a member of the Leadership Team, promoting positive problem solving among Leaders, nursing staff, and other personnel; maintaining acceptable standards of confidentiality and professional conduct.
  • Actively promotes and represents Milestone to the community, establishing and maintaining a good, professional relationship.
  • Actively participates in and assists with the development of the facility’s policies and procedures as necessary to ensure satisfactory individual services and acceptable staff conduct.
  • In compliance with Milestone, Inc.'s philosophy and policies regarding Human Rights, immediately intervenes on behalf of the individual(s), investigates individuals and/or staff complaints, grievances, and Human Rights Request for Interventions and provides timely adjudication and recommendations.
  • Functions as Supervisor-on-Duty on a rotating basis, in compliance with policies and procedures, and ensuring the safe functioning of the facility.
  • Provides back-up and assistance to SODs when needed.
  • Functions as Administrator-on-Duty when needed, in compliance with policies and procedures.
  • Provides 24-hour availability when acting as Administrator-on-Duty, ensuring the safe and efficient operation of the facility.
  • Prepares special reports and proposals as requested by the Senior Vice President.
  • Actively participates in the Quality Assurance Meeting.
  • Allocates consultant resources to best meet individual needs within budgetary constraints.
  • Actively seeks opportunities for continuing education units to fulfill requirements for licensure.
  • Performs other duties as required or assigned to ensure the safe and effective functioning of the facility.
