About The Position

Center for Human Development, Inc. is seeking a full-time Developmental Disabilities Service Coordinator to deliver case management services as defined and prioritized herein to individuals with intellectual/developmental disabilities; assist the program coordinator in monitoring the quality of services delivered within the county; and assist the program coordinator in the identification of insufficient service delivery resources and/or options.

Requirements

  • Bachelor's degree in behavioral science, social science, or a closely related field; or a bachelor’s degree in any field and one year of experience in human services; or an associate’s degree in behavioral science, social science, or a closely related field and two year’s work experience in human services; or Three years of equivalent training and work experience
  • Knowledge of the public service system for developmental disability services in Oregon.
  • Must possess a valid driver’s license
  • Pass all criminal history check requirements
  • Pass drug screenings.

Responsibilities

  • Identify access, coordinate and assure delivery of supports and services to individuals with an intellectual/developmental disability.
  • Determine eligibility for services as an individual with an intellectual/developmental disability per state guidelines.
  • Provides support and information to family members including individual consultation, support/education groups.
  • Assists clients in applying for and obtaining services and benefits.
  • Assist client with transportation if needed.
  • Assist client in community involvement.
  • Serves on advisory boards and committees concerned with intellectual/developmental disabilities.
  • Monitors client appointments and progress; maintains accurate case notes, treatment plans and client files; completes reports as required in a timely way.
  • Provides supportive and problem-solving counseling to clients and their families; responds to crisis situations.
  • Monitor delivery of services to individuals with intellectual/developmental disabilities.
  • Have the ability to work effectively as a member of a team.
  • Must maintain punctual and regular work attendance
