About The Position

ViaQuest Residential Services provides quality support and services to individuals with developmental disabilities in their homes. This part-time position requires weekend availability and involves providing direct care and support to individuals, ensuring their safety and well-being, and collaborating with a healthcare team.

Requirements

• Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program.
• LPN license.
• Valid driver's license, car insurance and reliable transportation.

Nice To Haves

• Nursing experience preferably within Private Duty Nursing, Home Health or Disability Services specialty is a plus but not required

Responsibilities

• Performing duties as outlined in the plan of care such as Medication Administration, Personal Care, Light Housekeeping and day to day support.
• 2:2 Ratio of individuals through a consistent caseload.
• Effectively communicating updates and changes to patient's status to the supervising RN.
• Maintaining a safe patient environment.
• Providing emotional and social support and compassionate care to patients.
• Maintaining current patient records and progress notes.
• Working as a member of the interdisciplinary team.
• Developing and establishing a plan of care with physician.
