Requirements
- DDS or DMD from an accredited dental school.
- Current Arizona dental license, or ability to obtain one prior to employment.
- Current CPR certification.
- DEA registration and NPI number, or ability to obtain as required.

Responsibilities
- Provide comprehensive general dentistry care.
