About The Position

As a Federally Qualified Health Center (FQHC), Community Health Center Inc. is proud to serve as an eligible practice site for clinicians participating in the National Health Service Corps (NHSC) Scholarship Programs and multiple HRSA loan repayment programs across Medical, BH and Dental. Community Health Center Inc. maintains strong HPSA scores of 19 for primary medical care, 21 for mental health, and 25 for dental services. Eligible providers who join Community Health Center Inc. may qualify for valuable financial assistance programs, including NHSC Loan Repayment, SUD Loan Repayment, Nursecorp LRP as well as the Connecticut Student Loan Repayment Program (CT SLRP)—which mirrors the HRSA-administered National Health Service Corps (NHSC) program and other HRSA supported workforce incentive initiatives designed to recruit and retain healthcare professionals dedicated to community-based primary care. Requirements

  • Must meet productivity targets quarterly
  • Must provide appropriate individualized patient education for maintenance of oral health.
  • Must emphasize preventive techniques and compliance with recall intervals.
  • Must provide radiographic services.
  • Must review patient medical history prior to start of visit.
  • Must maintain accurate documentation of dental hygiene services provided and a hygiene assessment including periodontal charting by completion of the electronic dental record at the end of each visit.
  • Must maintain the treatment area, equipment, instruments and supplies in proper working order daily.
  • Must strictly follow safety and infection control standards.
  • Must share responsibility for the maintenance of the log book required documentation and task list including the autoclave with the dental assistant at a fixed site.
  • Must ensure all electronic charting, documentation, and telephone encounters are in compliance with agency standards.
  • Must participate in assessment of dental services and quality assurance program.
  • Must maintain regular communication with the dental director, staff dentists and office manager.

Responsibilities

  • Provides oral prophylaxis, scaling and root planing, sealants, dental and periodontal charting and topical preventive treatments for patient population. To meet productivity targets quarterly
