About The Position

Quality Staffing, Inc., also doing business as Goddard Mason Executive Search, a leading full-service employment agency matching qualified talent with some of the area's top companies, is looking for an experienced Public Health Educator 2/Community Dementia Navigator to work through our agency as a contract employee with the Tennessee Department of Health (TN DOH) located in Hohenwald, TN. This is a hybrid position working from 8:00 am to 4:30 pm Monday through Friday and the pay rate for this position is $33.00 per hour. Dementia Navigators connect caregivers and individuals living with dementia to community resources and clinical support, helping them remain safely at home as long as possible. Responsibilities will include promoting dementia-friendly practices, providing memory screenings, education about Alzheimer's disease and related dementias (ADRD), encouraging early detection, improving access to services, and strengthening community-clinical partnerships. This position will be based in Lewis County and will serve Hickman, Lewis, Perry and Wayne Counties with some travel across Tennessee and will report to the Deputy Director, Office of Health Aging, Tennessee Department of Health.

Requirements

  • Graduation from an accredited college or university with a bachelor's degree in public health, nursing, social work or other comparable discipline and experience equivalent to two (2) years of full-time increasingly responsible professional public health, nursing, social work or another related field OR licensed practical nurses with five (5) or more years work experience working in a healthcare, home health, or community health.
  • Additional graduate coursework in public health, community organizing, social work, nursing, or other related fields may be substituted for the required non-specialized experience on a year-for-year basis.
  • Qualifying full-time experience in public health, nursing, social work, or other related fields may be substituted for the required education on a year-for-year basis, to a maximum of four (4) years.

Responsibilities

  • Communicating with persons outside the organization.
  • Building relationships with community partners to connect individuals with ADRD and caregivers to resources.
  • Delivering education materials, presentations, memory screenings, and resources to reduce dementia risk, supporting caregivers, and promoting dementia-friendly environments.
  • Collaborating with health care providers to promote early ADRD detection, diagnosis, resource awareness, and individualized care plans.
  • Distributing evidence-based educational materials and interventions to the community, individuals with ADRD, and caregivers.
  • Identifying and reporting funding opportunities to support local ADRD and caregiver activities.
  • Maintaining secure client information, tracking client outcomes and barriers to care, and streamlining referrals.
  • Monitoring program success, tracking individuals served, client satisfaction, and educational outreach.
  • Developing and tracking partnerships focused on dementia risk reduction and support for individuals with ADRD and caregivers.
  • Assessing and tracking community barriers to care for individuals with ADRD and caregivers.
  • Staying current on evidence-based practices in dementia risk, ADRD, and caregiving.
  • Utilizing data to inform program delivery and outreach.
  • Participating in ongoing training to stay informed on research and resources.
  • Communicating regularly with team members and supervisors at all levels to ensure program consistency.
  • Facilitating collaboration with co-workers and external partners to ensure effective support for individuals with ADRD and caregivers.
  • Communicating effectively with supervisors through multiple channels.
  • Maintaining and protecting personal and financial information.
  • Ensuring HIPAA compliance by securely storing and protecting client data.
  • Managing and tracking financial records according to state and county requirements.
