MECA Therapies is seeking a compassionate and motivated Delegated Care Coordinator to join our growing Maternal Health Program. As New Mexico's leading provider of community-based healthcare services, MECA is committed to improving health outcomes for mothers, infants, and families. Our Delegated Care Coordinators provide personalized care coordination, education, advocacy, and resource navigation for pregnant individuals and new mothers, helping them overcome barriers to care and achieve healthier pregnancies and births. The Delegated Care Coordinator provides comprehensive care coordination services to pregnant and postpartum members who may be experiencing medical, behavioral, or social risk factors. Working closely with healthcare providers, community partners, and families, this position ensures members receive timely prenatal care, postpartum support, education, and access to essential community resources. Services are delivered through a combination of home visits, virtual visits, telephone outreach, and community-based care coordination.
