DDD Day Habilitation Program Director
About The Position
Grace Community Care & Homes is developing its Developmental Disabilities and Autism department and seeks a DDD Day Habilitation Program Director. The role will contribute to person-centered services, reliable operations and accurate documentation. This is a planned role. Start date and duties are contingent on applicable program approvals, funding, credentialing and staffing readiness. Services may begin only within the approved scope. This is a part-time role. The final assignment and weekly schedule will be discussed during hiring. Clinical services begin only at approved locations and within the program's authorized scope.
Requirements
- IDD program leadership experience and qualifications matching the final DDD day habilitation model; ability to implement required policies and staff training.
- Applicants should provide relevant credentials, experience, availability and a resume.
- Final regulatory eligibility will be verified before appointment or service delivery.
Responsibilities
- Develop and oversee day-program operations, staffing, training, participant plans, safety, incidents, attendance and service documentation; coordinate with families and support coordinators.
- Work collaboratively with department leadership, intake, credentialing, billing and quality teams.
- Maintain confidentiality, respect participant choice, communicate service gaps promptly, and follow role-specific policies and professional scope.
- Participate in orientation, competency review and quality improvement.
