Then Prince William County Community Services (CS) is looking for you. Located within easy commuting distance of Washington, D.C., Northern Virginia, and Stafford County, Prince William County is a diverse and dynamic community. We are seeking qualified applicants to fill a full-time Service Coordinator III (Senior Clinical Services Caseworker) supervisory position in our Developmental Disabilities Services Program. Successful candidates will have experience providing supervision and quality assurance reviews, case management, completing assessments and individual service plan development, and have good verbal and written communication skills, team leadership and facilitation skills, and organizational skills to meet documentation and performance metrics while utilizing critical thinking to enhance successful outcomes for individuals with developmental disabilities, including those with co-occurring disorders. This position qualifies for a $5,000 hiring bonus. This bonus will be paid in two equal installments. The first installment of $2,500 will be paid after you have completed 90 days of employment. The final $2,500 installment will be disbursed upon successful completion of the probationary period. Requirements

  • Bachelor’s degree in psychology, social work, counseling, or a related behavioral science field
  • 3 years of experience providing service coordination and case management to individuals with developmental disabilities
  • Must possess a valid driver's license
  • Must pass DMV record check
  • Must pass TB test
  • Must pass a fingerprint-based national criminal history record check
  • Must pass a search of the registry of founded complaints of child abuse and neglected maintained by the Department of Social Services.

Nice To Haves

  • 1 year of supervisory experience
  • Experience providing services through Medicaid State Plan Option and Developmental Disability Waivers
  • Knowledge of person-centered practices, developmental disabilities, and dual diagnoses (DD/MH)
  • Experience using electronic health records; Credible preferred
  • Experience using WaMS for DD Waiver recipients

Responsibilities

  • Supervise up to 4 staff and maintain a caseload of individuals in need of or receiving services from community providers or other CS programs.
  • Perform difficult professional work designed to provide case management services including conducting intakes, eligibility determination, needs assessment, and service plan development.
  • Link, monitor, refer, coordinate, and facilitate services needed by the individuals.
  • Conduct and utilize assessments for individual service plan development.
  • Coordinate and facilitate individuals’ treatment team meetings.
  • Implement individual service plans and revise the service plan as needed/required.
  • Monitor quality of care through home and community visits to assure health and safety risks are addressed.
  • Document all services, contacts, and attempted contacts with individuals and team members.
  • Write individual service plans, progress notes, and quarterly service plan reviews.
  • Maintain annual trainings and provide in-service trainings.
  • Coordinate and lead interdisciplinary team meetings to develop and monitor service plans and delivery of services including service authorizations and private vendor authorizations.
  • Analyze data and prepare administrative reports.
  • Ensure proper adherence to federal, state, local, and agency regulations and licensure standards.
  • Link individuals with appropriate community resources and advocate for individuals in the community to include completing service plans, CANS, and presentation at FAPT/FAST meetings.
  • Provide consultation as emergencies arise and crisis intervention/supportive counseling for individuals and their families/community providers.
  • Serve on CS program committees with providers and/or administrators.
  • Organize and maintain individuals’ electronic health records to include updating data for the performance contract with DBHDS and dashboard measures.
  • Develop cooperative relationships with community groups and agencies.
  • Maintain ongoing communication with families, vendors, landlords, county programs and agencies, and state agencies to include care coordination agencies.
  • Provide supervision, monitoring, and evaluation of direct supports’ performance metrics, work quality, and documentation timeliness and assign cases.
  • Complete performance evaluations and implement performance improvement plans.
  • Provide case consultation to Clinical Services Caseworkers.
  • Complete utilization reviews and assist staff in prioritizing workloads.
  • Update status on waitlists, including statewide waiver waitlist, complete SARFs, and make referrals for services as needed.
