This role involves conducting comprehensive client assessments, participating in service plan development, and arranging for the delivery of needed services. The Case Manager will assist clients in accessing services, monitor their progress through referrals and follow-ups, and perform periodic reassessments. Advocacy activities on behalf of the client, such as writing letters for entitlement payments or encouraging self-advocacy, are also key responsibilities. The position requires maintaining case records in accordance with SCS and regulatory standards, providing case consultation to service providers, and communicating with supervisors regarding case management issues, resourcing, and service collaboration. Additionally, the role includes providing psychosocial education to clients and families to enhance their understanding of the client's condition and performing crisis assistance, including intervention and emergency referrals. The incumbent may also serve as a Primary Case Coordinator (PCC), with the scope of this role dependent on the individual's skill and autonomy. All staff are expected to exercise principle-centered leadership, focus on customer service responsiveness, and maintain a continuous quality improvement orientation, along with developing a working knowledge of and adhering to safety management policies and Joint Commission standards.
