About The Position

This position conducts and documents comprehensive assessments of clients and/or client’s needs. The Case Manager participates in the development of the client's service plan, consistent with the assessment, and arranges for the delivery of needed services. They assist clients in accessing needed services through referrals or linkages, and monitor client progress by making referrals, tracking appointments, performing follow-up on services rendered, and performing periodic reassessments of the client’s changing needs. Advocacy activities are performed on behalf of the client, such as writing letters to expedite entitlement payments or encouraging client or family members to advocate on their own behalf. Case records are prepared and maintained, documenting contacts, services needed, reports, and client progress in accordance with SCS and regulatory standards. Case consultation is provided, consulting with service providers/collaterals in determining client’s status and progress. Communication with the supervisor occurs regarding issues related to case management, resourcing, service collaboration, and development of new resources. Psychosocial education is provided to clients and/or family members to increase their understanding of the client’s illness, physical condition, or social situation. Crisis assistance is performed, including intervention on behalf of the client, making arrangements for emergency referrals, and coordinating other needed emergency services. The incumbent may serve as a Primary Case Coordinator (PCC), with the volume and complexity of PCC assignments depending on the incumbent’s skill and autonomy. Requirements

  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.
  • Minimum of zero year’s experience in case management/service coordination with a Bachelor’s degree.
  • Working knowledge of mental and physical characteristics of assigned population group (e. g., growth and development), human behavior, treatment/service options, and related issues.
  • Working knowledge of SCS programs and services, policy, and procedure.
  • Working knowledge of applicable regulations and licensure standards.
  • Good interpersonal skills and written and oral communication skills.
  • Reliable transportation for frequent travel.
