The Program Manager serves as the secondary leader of the Day Support team and plays a critical role in the lives of the individuals we serve. In this position, you will be responsible for developing, overseeing, and continuously improving Individual Support Plans (ISPs) to ensure each person receives the care, services, and support needed to achieve their personal goals and desired outcomes. Requirements

  • Bachelor's degree in a Human Services field or equivalent combination of education and experience
  • Demonstrated experience working with individuals with developmental disabilities and providing related services
  • Must maintain all required state and company training certifications
  • Ability to develop and deliver presentations
  • Strong professional written and verbal communication skills
  • Ability to effectively interact with government officials and diverse community stakeholders
  • Proven ability to motivate teams and manage multiple projects simultaneously

Responsibilities

  • Review assessments to identify supports and services aligned with each individual's goals and desired outcomes
  • Participate in shared planning by helping individuals communicate their needs and service requests
  • Monitor supports through assessments, interviews, and direct observation to ensure goals are being addressed
  • Make ISP modifications based on individual feedback and changing needs
  • Report progress and significant changes to Support Coordinators on a quarterly basis, including service appropriateness reviews and recommendations
  • Maintain Comprehensive and Electronic Service Documentation records for all individuals supported
  • Review and approve all documentation including PCRs, Part V documents, and ISPs
  • Conduct quality control audits on case files, service records, and program notes
  • Address documentation deficiencies with corrective action plans
  • Ensure all records comply with HIPAA and organizational privacy policies
  • Draft and implement responses to quality assurance audits, surveys, and regulatory agency findings
  • Stay current on Cornerstone policies, service manuals, and applicable regulations
  • Provide supervisory coverage in the absence of the Administrator, including interviewing, hiring, training, performance appraisal, and disciplinary actions
  • Assist with staff training on new and changing support practices for timely and accurate implementation
  • Work closely with the Administrator and Direct Support Staff on performance management
  • Attend and contribute to quality management team meetings
  • Participate in formal and informal investigations into incidents or allegations of abuse, neglect, or exploitation
  • Review incident reports to confirm appropriate treatment, injury causation, and preventative measures
  • Perform medication administration observations, mealtime monitoring, environmental assessments, and other required monitoring
  • Practice universal medical precautions utilizing personal protective and safety equipment as needed
  • Facilitate and strengthen relationships among the company, parents/guardians, and Support Coordinators
  • Work collaboratively with clinical professionals and external stakeholders
  • Obtain consents as required
  • Ensure confidentiality of sensitive information, including employee and service user privacy rights and protected health information
