About The Position

The Day Habilitation Supervisor will align his/her scope of work with Jawonio’s Mission while incorporating Jawonio’s Core Values as the guiding principle for all work activities. The Day Habilitation Supervisor will supervise the development, implementation, and scheduling of activities for the Day Habilitation program as assigned. This role may also oversee Prevocational and Respite Services, ensuring that individuals in the Day Habilitation Services receive appropriate day services. The supervisor will participate in conducting activities for these individuals and supervise staff within the program.

Requirements

  • Master’s degree or bachelor’s degree in a related field.
  • Five years’ experience leading activities, writing plans, and/or supervision of intellectually disabled or physically/multiply disabled individuals.
  • Must qualify as a Qualified Intellectual Disability Professional (QIDP) as per OPWDD.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write comprehensive reports and documentation.
  • Ability to speak effectively before groups of customers or employees of the organization.
  • Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
  • Knowledge of MS Word, Excel, and internet software.
  • A valid NY State Driver’s License and ability to drive with a driving record acceptable for agency insurance coverage.
  • Remain current in all OPWDD and/or OMH, as well as agency required training.
  • Candidates must be cleared and maintain acceptable records under the NYS OPWDD and/or OMH mandated criminal background check process.
  • Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
  • CPR and First Aid training before completion of the evaluation.
  • SCIP training may be required, as indicated by site location.
  • Medication Administration Certification (including G-tube, Insulin, and other subcutaneous injections) may be required as indicated by site location.
  • American Sign Language may be required, as indicated by site location.

Responsibilities

  • Supervise activities of Program Staff, including managing and appraising staff performance, formal and informal training, and orienting new employees.
  • Develop, implement, and schedule program activities and staff assignments, including volunteer opportunities, community integration, self-advocacy, disability issues, and personal development.
  • Participate with individuals and staff in activities as indicated.
  • Provide ongoing guidance and direction to staff, and train staff in appropriate intervention techniques and DSP Competencies.
  • Develop and implement staff action plans utilizing the HCBS waiver key of allowable services.
  • Review Regulatory Tracking Spreadsheets to ensure compliance and quality indicators are met.
  • Coordinate transportation services, assign drivers, coach drivers on participant support during transit, and ensure appropriate staff are assigned to runs requiring PRN support.
  • Serve as “Fire Marshall” for assigned sites, responsible for coordination of drills, evacuations, training, and documentation.
  • Review fire evacuation and other emergency procedures with direct reports, assign staff roles for evacuation, and update fire evacuation plans.
  • Formulate Staff Action Plans, conduct individual assessments, complete assessment summaries, progress notes, and data collection.
  • Chair team meetings and present Staff Action Plan progress and goals for the program.
  • Review and certify staff action plans and monthly reviews for quality indicators and billing requirements.
  • Monitor and assign DSPs for daily data collection responsibilities and check entries for compliance/completeness.
  • Generate and review billing reports for the program weekly and submit them to finance.
  • Complete or assign daily attendance calls for absent individuals who did not call in.
  • Serve as lead in the reporting of incidents and follow up with Quality Improvement and the Program Director.
  • Work with individual’s special needs during training activities to ensure safety, promote goal attainment, and provide role models.
  • Perform procedures as needed to fulfill each individual’s plan.
  • Assist other staff members in taking individuals on and off vans, providing physical support as needed.
  • Serve as liaison with families, parents, and group homes.
  • Ensure the program area is clean and safe, and that supplies, equipment, and materials are in good supply, stored correctly, and in good repair.
  • May be required to work remotely and use technology or alternative means of instruction to provide remote services.
  • May be required to prepare and implement lesson plans/activities and adjust instruction to accommodate individual needs via virtual sessions.
  • Participate in meetings regarding program issues and/or projects of the agency.
  • Perform a variety of clerical and administrative functions.
  • Provide new individual orientation and conduct tours of the program.
  • Perform ADL/personal care activities for individuals, as well as behavioral interventions, as needed.
  • Provide transportation services by driving the agency van as needed.
