About The Position

The Day Habilitation Coordinator is responsible for the development, coordination, and oversight of individualized program plans for each individual served by Upstate Caring Partners’ Day Habilitation programs who does not reside in an agency-operated residence. The Habilitation Coordinator will serve as treatment team leader conducting and coordinating all interdisciplinary team meetings, monitoring all services, ensuring compliance and acting as liaison with care coordination care managers.

Requirements

Must be a Qualified Individual Disability Professional (QIDP) as required by regulation by meeting the below requirements:
A bachelor's degree from an accredited program in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling, and psychology) as well as any related academic disciplines associated with the study of human behavior, human skill development, humans and their cultural behavior, or any other study of services related to basic human care needs or the human condition, or who holds permanent certification from the New York State Education Department in a field of education
Specialized training or at least one year of experience in treating or working with persons with developmental disabilities.
Travel is required.
Must have valid NYS Driver’s License.

Responsibilities

Serve as Treatment Team Leader, coordinating services and supports with all members of the treatment team, including Nursing, Behavioral Services, Therapy, families, and outside agency personnel.
Develop, coordinate, and monitor each resident’s day habilitation staff action and person-centered program plan, ensure completion of monthly summaries and required widget reviews.
Train staff on all service plans and associated documentation.
Ensure implementation and integration of services across the individuals’ programs
Monitor the services provided to individuals through direct observation in each service setting.
Complete monitoring visits during critical hours when people are in their program.
Complete daily/ weekly / monthly quality assurance reviews of day habilitation staff action plans and associated documentation. Provide follow up as needed with staff on documentation, service delivery, or other issues as they arise.
Maintain program participants’ active charts ensuring documentation is current and supports regulatory requirements.
Participate in leadership activities including meetings, trainings and committee involvement.
Ensure positive and effective communication and cooperation within and between department, division, agency, and community.
Serve as point person, liaison with people’s family and other natural supports
Establish a work environment that creates positive communication between supervisors and employees and assimilates new employees to the Agency’s culture, values and mission.
Ensure compliance with all pertinent government and agency regulations and operating standards
