About The Position

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery. Prototypes DMH Outpatient’s Children and Youth Behavioral Health Initiative (CYBHI) aims to enhance mental health services for children and adolescents, particularly in underserved communities. The program focuses on improving access to care, integrating behavioral health with primary healthcare, and ensuring services are culturally competent and evidence based. Emphasizing prevention and early intervention, the initiative seeks to address mental health issues before they escalate. By fostering collaboration among schools, community organizations, and healthcare providers, the program creates a supportive network that promotes mental wellness, resilience, and positive outcomes for youth. The Data Coordinator plays a vital role in the treatment team, focusing on administering and quantifying treatment outcomes through client interviews. This includes conducting interviews at the start, midpoint, and end of treatment to assess changes in behavioral health, the effectiveness of services, and overall program outcomes. Key responsibilities include collaborating with team members to coordinate the administration of outcome measures and entering data into relevant systems to meet grant and public funding requirements.

Requirements

  • High school diploma or equivalent required.
  • Current First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Two years’ experience in data collection and entry preferred.
  • Two years’ experience in an administrative support role.
  • One year experience working in a mental health setting preferred.

  • Bachelor’s degree preferred.
  • Experience working with children and caregivers preferred.

Responsibilities

  • Engage with clients at key intervals (intake, 6-month mark, discharge) to conduct clinically and culturally appropriate interviews.
  • Document interviews and complete outcome measures within 2 business days of intake.
  • Enter outcome measure data into relevant portals.
  • Ensure timely and accurate billing documentation.
  • Seek clinical consultation as needed, especially in high-risk situations.
  • Collaborate with the treatment team, including Therapists, Occupational Therapists, Case Managers, and the Psychiatric/Medical Team.
  • Provide services in optimal physical locations based on client needs.
  • Assess case management needs and handle necessary releases of information.
  • Assist clients in connecting with resources and linkages.
  • Write and complete progress notes in line with agency guidelines after service delivery.
  • Ensure all clinical documentation is submitted on time and complies with agency policies, HIPAA, 42-CFR, DMH, and other regulatory standards.
  • Draft client progress letters and court reports, obtaining supervisor approval.
  • Complete all assigned URCs promptly.
  • Perform administrative tasks: data entry, scanning, filing, organizing, faxing, mailing, and client management.
  • Assist and direct calls and clients to the appropriate staff or departments, check in clients for appointments and groups.
  • Participate in agency and team meetings actively.
  • Engage in training opportunities and complete all assigned training on time.
  • Read and respond to emails when appropriate.
  • And perform other duties as assigned.
