About The Position

Under the general supervision of the Children and Youth with Special Health Care Needs (CYSHCN) Systems Development Group Manager, the Children and Youth with Special Health Care Needs (CYSHCN) Community Resource Coordinator performs highly complex (senior-level) work requiring demonstrated knowledge of contract management, including consultative services and technical support related to the planning, development, and implementation of systems of care for CYSHCN, youth with disabilities, and their families. Oversees, manages, and coordinates CYSHCN statewide community-based contractors. Develop and manage contracts, work plans, and quality assurance and improvement activities. Activities support essential public health services and efforts to achieve the Title V Maternal and Child Health Block Grant performance measures related to CYSHCN and their families in Texas communities. Coordinates, develops, and implements the Title V Block Grant action plan. Independently performs associated planning and collaborative activities with other agency programs, organizations, and groups that address issues pertaining to CYSHCN. Works under limited supervision, with considerable latitude for initiative and independent judgment.

Requirements

• Knowledge of professional public health principles and contract practices, including trends, development, and quality improvement methods, and their application to the care of CYSHCN and their families.
• Knowledge of systems of care and support for CYSHCN, their families, and communities.
• Knowledge of CYSHCN service systems including services provided by other state agencies and private entities.
• Knowledge of federal and state laws and mandates which govern the delivery of services for CYSHCN and their families.
• Knowledge of group process, leadership and communication techniques.
• Knowledge of principles of planning.
• Knowledge of contract bidding, development, implementation, monitoring, and quality assurance.
• Skill in assessing, planning, and implementing health care services contracts especially as related to CYSHCN and their families.
• Skill in organization and in directing, coordinating, monitoring, and evaluating diverse group or team activities.
• Skill in organizing and conducting formal meetings/conferences.
• Skill in the use of computers.
• Skill in communicating information clearly, concisely, and effectively, verbally and in writing.

• At least two years of experience in project reporting, presentation, and developing contract tracking materials.

• At least two years of experience with CYSHCN programming and policy development.

Responsibilities

• Leads and manages comprehensive program contract oversight for the CYSHCN systems development group contracted community initiatives with considerable authority for contract decisions regarding the details of contract development, planning (workplans with short and long-term goals), implementation, quality assurance and improvement activities, reporting, evaluation, networking, and coordination.
• Leads the development of contract request for proposals (RFPs) for program initiatives and CYSHCN application review and/or contract renewal processes.
• Negotiates contracts with selected community organizations and consults with internal stakeholders and regional staff throughout the process.
• Responsible for CYSHCN contract budget development and funding proposals.
• Reviews and evaluates CYSHCN contract service delivery methods, outputs, and activities; identifies contract resource gaps; and makes recommendations for contract improvement and funding based on review criteria.
• Schedules and performs CYSHCN contract quality assurance monitoring visits as needed.
• Responsible for the development of CYSHCN contract quality assurance monitoring materials, preparation of on-site visits or desk reviews, and development of evaluation reports.
• Leads the annual CYSHCN contract review of standards and the initial development of standards.
• Performs CYSHCN contractor’s programmatic risk assessments for assigned initiatives.
• Plans and coordinates activities for specific Maternal and Child Health Section CYSHCN community-based initiatives that support Title V mandates, promotes Title V CYSHCN Performance Measures progress, and collaborates with other Title V Maternal and Child Health staff, other DSHS staff (central office and regional), providers, and families.
• Establishes program guidelines, goals and objectives, procedures, standards, and timelines.
• Provides consultation and technical assistance to providers, contractors, and DSHS regional staff.
• Identifies needed areas of change and makes recommendations to evaluate and improve program activities, including contractor activities.
• Works with stakeholders in assessing service needs and gaps.
• Supports the development and/or redesigning of systems of care.
• Collaborates with program staff in the planning, development, and implementation of new initiatives and the development of tools and instruments for collecting and analyzing data.
• Reports on these activities for the CYSHCN Systems Development Group.
• Establishes effective partnerships and collaborative workgroups with other state agencies, disability service/advocacy organizations, consumer and provider groups, funding entities, and internal partners at DSHS to further the awareness of the needs and issues of children, youth, and adults with special health care needs and their families and collaboratively plan activities to address Title V CYSHCN Performance Measures.
• Represents the CYSHCN Systems Development Group, Maternal and Child Health Unit, and Department of State Health Services at meetings, hearings, conferences, and seminars or on boards, panels, and committees.
• Promotes the use of evidence-based practices.
• Seeks opportunities to integrate services for establishing medical homes, health care transition, and community inclusion for CYSHCN and their families.
• Independently researches program policy/budget impact of services provided, systems changes, and grant proposals, including analysis of data from contractors and other state and federal programs, legislation, and strategic planning.
• Tracks and reports on progress toward strategic planning objectives.
• Develops and disseminates educational/outreach publications.
• Contributes to and confirms the accuracy of the content on the CYSHCN Systems Development Group website.
• Determines training needs.
• Develops and may deliver educational/training tools, presentations, and curricula on issues pertaining to CYSHCN to providers (including contractors), families, and staff through various formats.
• Confirms stakeholder involvement in educational/training material development and dissemination.
• Actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery, Continuity of Operations (COOP) activation, and/or serving on the State Medical Operations Center (SMOC) team.
