Under the general supervision of the Children and Youth with Special Health Care Needs (CYSHCN) Systems Development Group Manager, the Children and Youth with Special Health Care Needs (CYSHCN) Community Resource Coordinator performs highly complex (senior-level) work requiring demonstrated knowledge of contract management, including consultative services and technical support related to the planning, development, and implementation of systems of care for CYSHCN, youth with disabilities, and their families. Oversees, manages, and coordinates CYSHCN statewide community-based contractors. Develop and manage contracts, work plans, and quality assurance and improvement activities. Activities support essential public health services and efforts to achieve the Title V Maternal and Child Health Block Grant performance measures related to CYSHCN and their families in Texas communities. Coordinates, develops, and implements the Title V Block Grant action plan. Independently performs associated planning and collaborative activities with other agency programs, organizations, and groups that address issues pertaining to CYSHCN. Works under limited supervision, with considerable latitude for initiative and independent judgment.
