About The Position

The Cardiovascular Clinical Certification Program Specialist role provides guidance and operational direction in the coordination of the continuum of cardiovascular care, ensuring alignment across departments, improving system performance, and achieving high-quality patient outcomes. This role works in collaboration with the program director, physician leadership, nursing, and multidisciplinary teams to drive performance improvement, regulatory compliance, and program effectiveness.

Requirements

  • Expert knowledge and ability sufficient to develop and apply procedures in complex assignments
  • Ability to provide clinical and community education
  • Excellent computer skills with the experience in data collection, interpretation and analyzing. May include assisting with other types of facility collection of data
  • Professional knowledge and technical ability to analyze, design, test and implement systems and applications
  • Ability to work independently while having sound judgment and attention to detail
  • Ability to handle multiple tasks simultaneously
  • Bachelor's degree or higher from an accredited school of nursing
  • License (must meet one of the following): Current Wyoming Registered Nurse (RN) License from the Wyoming State Board of Nursing OR Current enhanced Nurse Licensure (eNLC)
  • Four (4) or more years of cardiovascular, emergency or critical care nursing experience
  • 14 Days: Cheyenne Regional AHA RQI within fourteen (14) calendar days of start date

Responsibilities

  • Ensures a high-quality and comprehensive continuum of care for patients starting at admissions and continuing after discharge.
  • Oversees the comprehensive planning, educating, advocating and coordinating care through admission to discharge and follow-up. Participates in the development of disease specific systems of care at the community, state and regional levels.
  • Acts as a liaison between patient care team, specialty services, and community or external resources to coordinate and maximize resources and identify and resolve barriers to the plan of care.
  • Partners with physician leadership, unit leadership, and the Clinical Education Department to plan, design, develop, implement, and evaluate disease specific education and support programs for staff and providers across the hospital. Helps plan patient support group for cardiovascular disease specific programs.
  • Monitors clinical processes, outcomes, key cardiovascular metrics and system issues related to the quality of care provided, works with director to develop new quality measures to identify trends and assist in developing corrective activities and monitors. Reports program performance and outcomes to director and committees, physician directors, and staff.
  • Reviews cases and maintains local disease specific committee structure(s) and provides ongoing support to committee members.
  • Collaborates with Data Management Department to ensure timely and comprehensive data collection and develop processes for validation of data. Takes an active role in facilitating performance improvement activities, trend reports and research.
  • Plans and implements practice management guidelines and clinical protocols. Monitors care of in hospital patients, serving as a resource for clinical practice. Participates in facilitating case reviews, monitoring, and follow-up.
  • Promotes alignment on evidence-based practices and program goals.
  • Performs surveillance and researches upcoming or challenging quality measures affecting disease specific care for quality and pay for performance initiatives.
  • Manages and coordinates all activities to achieve and maintain Joint Commission certifications or similar framework for disease specific program success.
