About The Position

Endeavor Health Services is seeking a full-time Critical Time Intervention (CTI) Case Manager to support individuals during critical life transitions, such as moving from hospitalization or higher levels of care back into the community. This role involves providing focused, time-limited support to help individuals secure housing, access services, and build community connections. The Case Manager will work collaboratively within a team to equip individuals with the necessary tools and support for long-term stability and independence.

Requirements

Bachelor’s degree in social work, psychology, or related field
2+ years of case management or related experience
Valid driver’s license and reliable transportation
Flexibility for occasional evenings or weekends as needed
Working knowledge of Microsoft Office

Responsibilities

Conduct comprehensive assessments of client needs, strengths, and goals
Develop individualized, short-term support plans focused on stability and transition
Coordinate transitions from hospitals or other settings into the community
Collaborate with healthcare providers, social services, and community organizations
Connect people to resources including housing, employment, and healthcare
Build strong, trusting relationships to encourage engagement in services
Monitor progress and adjust plans to support successful outcomes
Advocate for client needs across systems and services
Maintain accurate, timely, and compliant documentation
Participate in team meetings, training, and ongoing professional development
Identify and address barriers to long-term stability
Provide education and support to the people we serve and families
