About The Position

The purpose of this position is to provide on-site, in-person intervention for individuals of all ages (adults and children) experiencing a mental health crisis. Intervention includes conducting mental health assessments to determine level of care needed for individual, making referrals for individual and/or family to appropriate medical, mental health, and/or community resources and transporting as appropriate to higher level of care. Assist in diverting target populations from the criminal justice system back into treatment services.

Requirements

  • High School Diploma or GED is required
  • Must possess a valid driver’s license and be able to meet driver eligibility guidelines.
  • Must be in good physical and mental health and pass a complete physical exam prior to employment (to include PPD).
  • Must have basic knowledge of MS Office computer software.
  • Must possess the ability to maintain accurate written daily, weekly, and monthly reports.
  • Must perform all duties in a manner that ensures the strictest confidentiality due to access to sensitive information.
  • Must possess the ability to read, write, and speak the English language fluently.
  • Must be able to continually and effectively employ professional verbal and written communications skills.
  • All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.

Nice To Haves

  • preferred two years of experience in behavioral or mental health services.
  • Must complete 30 hours of crisis and mental health in-service training (in addition to the required 30 hours of department approved training) within one year of becoming employed.

Responsibilities

  • Demonstrate the Public Health Department's core mission, vision and values
  • Within parameters of safety protocol, have face-to-face contact with individuals in mental health crisis within the Winnebago service area.
  • Collect medical and mental health histories for individuals in mental health crisis.
  • Conduct initial crisis assessment screenings to determine risk and triage for appropriate level of care for individuals in mental health crisis.
  • Develop action and safety plans for individuals in mental health crisis to reduce crisis and maintain stabilization.
  • Coordinate and refer individuals in mental health crisis to other medical, mental health, and community resources, as appropriate.
  • Collect demographic data on each mobile crisis response.
  • Ability to effectively utilize office equipment to scan, fax, copy, and upload confidential information to electronic health record system.
  • Ability to maintain a safe, clean, and sanitary environment by adhering to health and safety policies and guidelines.
  • Maintain individual charts on each client following policies and regulations established by the agency.
  • Record timely, up-to-date progress notes for each contact with clients utilizing electronic health record system.
  • Participate in a paid weekend on-call rotation with other teammates.
  • Participate in an interdisciplinary approach to client care, complete internal program referrals, and demonstrate a high degree of team-based treatment.
