About The Position

The Crisis Mobile Response Clinician will triage incoming behavioral health/substance use crisis calls and deploy to community settings (i.e., family home, school, shelter, library, etc.) as needed in order to effectively and efficiently manage consumers in crisis. Crisis response may include but not limited to assess, engage, de-escalate, stabilize, safety plan, link and coordinate services, pre-screen, assessments, and hospitalization level of care decisions for Medicaid or indigent consumers of Monroe County based upon state guidelines for hospitalizations. The Crisis Mobile Response Clinician will also be responsible to ride along with law enforcement to assist with de-escalation of a behavioral health/substance use crisis as deemed appropriate. The Clinician will also interface with the community leaders, law enforcement and other providers.

Requirements

  • Bachelor’s Degree in Social Work with State of Michigan Licensure (LLBSW, LBSW) or Bachelor’s Degree in a related Human Services field.
  • Master’s degree in social work, psychology, or counseling with State of Michigan Licensure (Licensed Master’s Social Work, License Psychologist, Licensed Professional Counselor, Licensed Marriage and Family Therapist).
  • Two years previous experience with Mentally Ill or Co-Occurring (MI/Substance Abuse) populations preferred.
  • One year experience with children with SED and adults/children with a developmental disability preferred.
  • Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served.
  • Demonstrated knowledge of the clinical needs, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span.
  • Ability to assess data reflecting each individual’s status; interpret the appropriate information needed to identify each individual’s requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group - (adolescent, adult and geriatric).
  • Ability to work in a complex, detail oriented, deadline and structured environment.
  • Ability to communicate effectively in person and/or phone.
  • Ability to read, analyze and interpret information.
  • Ability to write correspondence and procedures.

  • Valid driver’s license, proof of current automobile insurance and driving record determined by the Authority’s insurance carrier to be eligible for coverage if required as part of positions duties.

Responsibilities

  • Triage incoming behavioral health/substance use crisis calls and deploy to community settings.
  • Assess, engage, de-escalate, stabilize, safety plan, link and coordinate services, pre-screen, assessments, and hospitalization level of care decisions.
  • Ride along with law enforcement to assist with de-escalation of a behavioral health/substance use crisis.
  • Interface with community leaders, law enforcement and other providers.
  • Identify severity of illness by assessing symptom acuity, functional abilities, and risk factors.
  • Accurately assess psychiatric symptomatology and risk of harm to self or others.
  • Assess, complete, and document an accurate behavioral health diagnostic impression diagnosis based on the current Diagnostic and Statistical Manual (DSM) and knowledge of corresponding ICD codes, and treatment recommendations.
  • Develop an evaluation of what is causing a crisis situation and develop a plan of safety, which includes alternative resources to hospitalization.
  • Perform pre-screening for admissions for psychiatric hospitalization.
  • Screen all incoming calls, providing crisis service for adults, children, adolescents and families.
  • Provide face-to-face assessments consistent with authority standards as needed.
  • Schedule admission appointments, or make referrals to outside agencies or service providers consistent with signed contracts and/or interagency service agreements.
  • Coordinate supports/services with the consumer’s identified primary health care provider and upon consent, send letter to the primary care physician according to agency standards.
  • Assess need for psychiatric consultation authorization.
  • Provide individual and group community education by phone and/or by group presentations.
  • Work in conjunction with other Access System staff to triage, coordinate the provision of appropriate crisis intervention, emergency service status program placements, follow-up, and after care, and to provide initial and continued stay level of care authorizations.
  • Assign consumers to appropriate Clinical Department of the authority based upon triage assessment and provide level of care authorizations for continuing care consistent with authority standards.
  • Maintain complete and detailed case records and service activity documentation consistent with MDHHS best practice guidelines and standards, authority procedures, and requirements of third-party payer sources.
  • Implement interdisciplinary peer review recommendations and decisions on a retrospective and prospective basis.
  • Complete assessments/pre-screens, determining appropriateness of emergency psychiatric hospitalization to community hospitals.
  • Respond to consumer requests for emergency services via phone or face-to-face contact consistent with authority standards in an effort to stabilize the crisis situation.
  • Complete assessments of children and adults to determine appropriateness of emergency psychiatric hospitalization.
  • Complete alternative emergency service status placements when assessments determine they are appropriate.
  • Perform other related duties as assigned.
  • Transport consumers when necessary.
  • Identify, operationalize, and implement, with approval, performance improvement indicators for this program area.
  • Actively participate in agency-wide performance improvement activities.
  • Maintain individual credentials and participate in education and training necessary to maintain current competency(s) in skill areas essential for implementing job duties and responsibilities.
  • Follow oral and written direction completing tasks as assigned by due dates.
  • Maintain working knowledge of software applications used in performance of daily duties.
  • Comply with and maintain competence with all applicable laws, regulations, standards, and rules governed under Medicare, Medicaid, Michigan Department of Health and Human Services (MDHHS), The Joint Commission, and any applicable funding sources.
  • Understand, adhere, and maintain competence with all MCMHA and CMHPSM standards, policies, procedures as they appear in MCMHA’s, Management of Information, Finance-Contract Management, Recipient Rights, Performance Improvement, Administration, Human Resources, Environment and Provision of Care Policies.
  • Comply with and maintain competence with the Health Insurance Portability and Accountability Act (HIPAA) regulations as they pertain to privacy and confidentiality.
  • Interact appropriately with managers and peers.
  • Perform required duties with an awareness of impact upon the Authority structure as a whole.
  • Work as a Team Player within the entire Authority Structure.
