About The Position
The Crisis Care Coordinator - REACH position is part of the Youth Crisis Team in Milwaukee, WI, with Lutheran Social Services of WI & Upper MI. This role supports youth in psychiatric crisis through the REACH program, focusing on Reaching, Engaging, and Assisting Children & Families. The coordinator provides trauma-informed, person-centered care coordination to prevent out-of-home placements and connect youth with community resources.
Requirements
- Bachelor’s degree in human services (e.g., Social Work, Psychology, Counseling)
- Minimum 1 year of mental health service experience
- Valid driver’s license and reliable transportation
Responsibilities
- Conduct assessments and develop individualized service plans
- Facilitate care coordination and crisis intervention
- Collaborate with families, providers, and service teams
- Participate in after-hours on-call rotation
