About The Position

Work with the CN Guidance FACT Team to deliver services to individuals diagnosed with severe mental illness whose needs have not been met by more traditional service delivery approaches. This role involves integrating dual-recovery treatment, family support, or vocational/employment training with the tasks of other team members, with a lead responsibility for justice-involved clients. The specialist will also coordinate with various stakeholders to ensure compliance and facilitate successful reentry for consumers.

Requirements

Masters Degree in Counseling or related field.
LMSW, LMHC-LP, LMFT-LP, or LCAT-LP.
At least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.
Experience working with formally incarcerated clients and/or working with justice involved clients.
Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated.

Nice To Haves

Knowledge of violence risk assessments.
Knowledge of court procedures are a plus.

Responsibilities

Perform routine team duties and have lead responsibility for justice-involved clients.
Integrate dual-recovery treatment, family support, or vocational/employment training.
Complete required AOT, parole, probation, or court progress reports.
Escort consumers to related hearings and appointments.
Involve family, significant others, housing providers, defense attorney, and/or community corrections to ensure compliance and coordination of services.
Assist the ACT team and consumer in developing service plan goals and objectives that promote reductions in criminogenic risk and prosocial community living.
Coordinate with jail and prison reentry coordinators and visit consumers during detention to facilitate their safe and successful reentry.
Coordinate, advocate, and liaise with probation and parole officers.
Provide Assessment and Service Planning.
Participate in Quality Assurance and Improvement Processes.
Engage in Utilization Review.
Perform incident reporting and coordination between Forensic ACT and other systems.
Perform assessments, case management, crisis intervention, and discharge planning.
Maintain case records and complete documentation on time and in compliance with regulations.
Provide direct, face-to-face and/or collateral services to recipients in the office or community.
Perform periodic on-call coverage.
Assist recipients in developing appropriate expectations from treatment.
Strengthen the recipient's ability to understand and utilize therapy and find resources after treatment.
Utilize and understand the value of supervision.
Facilitate sharing of data with individuals to whom it applies.
Complete QI Training during onboarding and participate in QI refresher training as needed.
Cultivate a culture of inclusion for all employees.
Adhere to all safety protocols and procedures.
Demonstrate a commitment to maintaining a safe work environment.
