A full-time non-exempt position with primary working hours of Monday through Friday 8:30am-4:30pm (37.5 hours/week). Paid training provided. Services Provided through the CJS Program: Manages needs for those clients who are currently in County Prison or State Correctional Institutions and works with community providers, clerk of courts, public defender, etc. to work for the timely release of the individual. Advocates for medication routine and safety while inside the institution and provides referral assistance and linking to housing, entitlements, treatment and community activities upon release. Monitors and follows up with the individual at least bi-weekly, but in most circumstances, weekly. Educate the client on available resources and how to access them. Assist with goals to meet basic living needs and skills, such as housing, food, medical care, recreation, education, and employment. May accompany the client on appointments related to their physical and behavioral health. Direct involvement in discharge planning from state and community hospitals. Occasional, rotating on-call shift for after-hours crisis situations.
