Job Title: Care Manager I Schedule: Mon-Fri, 8:00a-4:30p Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available. Position Summary: Provide case management services to consumers with mental illness who are receiving services through the Criminal Justice Liaison program funded under the TN Department of Mental Health and Substance Abuse Services. The position responsibilities for this Criminal Justice Case Manager are in conjunction with the position responsibilities of the Criminal Justice Liaison(s) for the regions served.

Requirements

  • Bachelor’s Degree in a health-related field of counseling, psychology, social work or other behavioral health field. Candidates with a bachelor’s degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.

Responsibilities

  • Proactively identify and engage consumers who are seriously mentally ill (SMI) in the Criminal Justice System.
  • Identify service providers in various organizations, institutions and agencies serving or having contract with SMI consumers.
  • Develop community resources and coordinate existing resources for this population.
  • Meet with identified service providers to educate them about the Organization’s programs and to solicit their assistance in identifying consumers who would benefit from the program.
  • Monitor progress of consumers’ legal cases and conduct follow up activities to ensure that resources continue to meet the individualized need of the consumer.
  • Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
  • Conduct follow-up activities with consumers when released from jail into the community programs and services to ensure successful integration into the community from incarceration.
  • Complete all paperwork necessary, including imputing information into the TNWITS system.
  • Provide consultation to Outpatient Care Managers and other mental health providers regarding assessment evaluations, as needed.
  • Inform program personnel and/or community of trends, issues and concerns.
  • Ensure program protocols/policies and procedures are carried out.
  • Submit monthly and quarterly reports to TDMHSAS as indicated in grant requirements.
  • Provide support and consultation for Care Managers with consumers in crisis situations.
  • Ensure consumers are provided efficient transitions within the Agency and in the community.
  • Assist Outpatient Care Managers and other staff with referral and linkage to community resources and advocate for consumers when necessary.
  • Attend Court staffings and treatment team meetings to provide up-date and recommendations for Consumers.
  • Responsible for attending all mandatory in-services, continuing education and annual health requirements.
  • Comply with all Corporate Compliance policies, practices and procedures. Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director
  • Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator.
  • Will be compliant with the relevant regulations concerning the privacy and security of consumer’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Participate in proactive team efforts to achieve departmental and company goals.
  • Perform other duties as assigned.
  • Provide leadership to others through example and sharing of knowledge/skill.
