About The Position

Elk Ridge Community Health is seeking a flexible, mission-driven Coverage Clinician (NP/PA) for a uniquely dynamic role with no assigned patient panel. This hybrid position offers a meaningful way to support patient care across the organization while enjoying the balance of both in-clinic and telehealth work. The role involves providing high-quality patient care across North Country via in-person visits and telemedicine, with a focus on chronic disease management, preventative care, care transitions, and closing population-health gaps. The clinician will also provide in-person clinic coverage to support continuing service delivery during provider absences, manage electronic inboxes and desktops for clinicians on PTO or leave, assist during the onboarding of new clinicians, and collaborate with care teams to strengthen care coordination. Effective communication with patients from diverse backgrounds using telehealth tools and EHR is essential, as is contributing to organizational improvement through workflow-enhancement projects and meeting performance goals.

Requirements

  • Minimum of 2 years experience
  • A flexible and collaborative approach to supporting colleagues’ care practices.
  • Must have National Certification
  • Hold or be eligible to obtain an Arizona License and DEA
  • Must have a Level One Fingerprint Clearance card or fingerprint receipt before hire/transfer date, and card must be received within 30 days of hire/transfer date.
  • Undergo drug testing prior to employment and be subject to post-accident, reasonable suspicion, return to duty and follow up drug and alcohol testing.
  • Obtain and maintain vaccination status (i.e., as recommended by CDC and/or other public health agencies) to include an Influenza vaccination (subject to exemptions and accommodations).

Responsibilities

  • Provide high-quality patient care across North Country—via in-person visits and telemedicine—with a focus on chronic disease management, preventative care, care transitions, and closing population-health gaps.
  • Provide in-person clinic coverage to support continuing service delivery during provider absences.
  • Support system needs by covering electronic inboxes and desktops for clinicians on PTO, extended leave, or who have left the organization, ensuring continuity of care and timely follow-up for patients.
  • Assist during onboarding of new clinicians by managing short-term patient needs and helping bridge care gaps while insurance enrollment and panel development are in progress.
  • Collaborate with care teams, case management, and support services to strengthen care coordination and connect patients efficiently to appropriate resources.
  • Use telehealth tools and the EHR effectively to communicate clearly, compassionately, and in culturally sensitive ways with patients from diverse backgrounds.
  • Contribute to organizational improvement through workflow-enhancement projects and by meeting key performance goals, such as timely follow-ups on care gaps and rapid response to urgent results and needs.
