About The Position

The BridgeSPAN/Court Case Manager is responsible directly for providing quality client care in the medical and legal settings by overseeing the advocacy and documentation of each case. The Case Manager is a direct link between clients and the community.

Requirements

  • Bachelor's degree in Social Work, Psychology, Counseling or related field from an accredited college or university.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
  • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume.
  • Ability to apply concepts of basic algebra and geometry.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • CPR/First Aid Certification.
  • Must be a Medical Technician.
  • Ability to type and perform Data Entry/10 key.
  • Proficiency in computer software, including Windows, Word for Windows, Alice, and Access.
  • Must be at least 20 years of age.
  • Pass a background check.
  • Pass Family Care Safety Registry screening.
  • Pass fingerprint screen.
  • Pass drug screen.

Responsibilities

  • Assists residential clients in crisis.
  • Answers hotline calls and responds appropriately to callers.
  • Accesses client needs and misc. case management needs.
  • Attends regularly scheduled staff meetings and case reviews.
  • Attends training and workshops.
  • Maintains client confidentiality.
  • Documents client activities in client files and CCL.
  • Initiates and maintains Aftercare/Follow-up with all clients in BridgeSPAN & Court Services programs.
  • Participation in Performance and Quality Improvement activity is required by each employee.
  • Acts as liaison between clients and collaborating hospitals.
  • Trains medical staff to screen patients for domestic violence.
  • Provides face-to-face advocacy services to victims of domestic violence seeking medical care, including crisis intervention and support, safety planning, assistance in filing protection orders, access to emergency shelter, information and referral to resources.
  • Provides face-to-face advocacy to hospital and clinic personnel who are victims of domestic violence.
  • Assists clients in locating assistance for housing, utilities and medical expenses, employment search and searching for affordable daycare.
  • Identifies individual transportation options.
  • Assists clients with telephone calls.
  • Collects data on the number of medical staff receiving training; the number of victims of abuse identified by hospital or clinic staff; the number of people who receive advocacy services, and the number of hospital or clinic staff receiving advocacy services.
  • Enters data into the computer system.
  • Answers pages and follows up on referrals.
  • Communicates with staff, medical personnel, and law enforcement.
  • Attends and facilitate monthly/quarterly domestic violence task force meetings.
  • Provides information and support in court to domestic violence victims on municipal, civil and criminal cases as appropriate; provides order of protection paperwork services if assigned to an ex-parte docket.
  • Provides information, support and follow up services to survivors contacted at court by phone and hearings.
  • Keeps abreast of new legislation and changing ideas in the area of domestic violence and court advocacy.
  • Acts as a liaison with legal community partners and KCPD partnership.
  • Coordinates KCPD Lethality Assessment Program.
  • Acts as On-Call Supervisor and in on-call rotation for the healthcare setting and at shelter.
