About The Position

Empowered Services is seeking a County Coordinator to assist with county coordination duties while also working part-time as a Direct Support Professional (DSP). This blended role requires balancing communication, documentation, service coordination, provider support, and direct care. The County Coordinator will support assigned customers and providers by conducting check-ins, assisting with meet-and-greets, updating support documents, communicating with Services Coordinators and Personal Agents, and ensuring services are consistent and well-supported. This position is ideal for individuals with DSP, caregiving, care coordination, case management support, or customer service experience who are ready for increased responsibility.

  • Ability to document clearly, professionally, and accurately.
  • Ability to manage multiple tasks and follow-up items.
  • Ability to remain calm and solution-focused during sensitive situations.
  • Comfort using phones, email, text communication, documentation systems, and basic computer programs.
  • Reliable transportation and ability to travel locally as needed.
  • Ability to provide direct support services as part of the role.
  • Commitment to person-centered support for individuals with intellectual and developmental disabilities.
  • Background check required due to access to vulnerable individuals.

Nice To Haves

  • Experience as a DSP, caregiver, coordinator, case management assistant, or similar role.
  • Experience supporting individuals with intellectual and developmental disabilities.
  • Experience communicating with families, guardians, Services Coordinators, Personal Agents, case managers, or provider agencies.
  • Experience completing service notes, progress updates, reports, protocols, or support plans.
  • Knowledge of Oregon IDD services, counties, brokerages, or in-home and community-based supports.

Responsibilities

  • Coordinate Customer and Provider Support: Conduct regular check-ins with assigned customers and providers, respond to communications promptly, follow up on concerns and needs, assist with customer and provider matching, coordinate meet-and-greets, and identify service gaps.
  • Maintain Documentation and Support Plans: Document interactions, maintain accurate demographic information, collect monthly updates, assist with reports and action plans, obtain necessary approvals, and send documents to relevant parties.
  • Support and Train Providers: Train providers on customer-specific needs, answer questions, coach on professional conduct, and communicate provider concerns to supervisors.
  • Problem-Solve and Escalate Concerns: Respond to concerns calmly, gather information, document issues, support respectful communication, and escalate serious concerns to leadership.
  • Provide Direct Support: Work scheduled DSP shifts, support customers with daily living tasks and goals, follow customer-specific protocols, complete service documentation, and promote customer dignity and independence.
